针刺辅助镇痛在无痛分娩中的应用及对外周血MAPKERK信号通路相关分子表达水平的影响

The application of acupuncture-assisted analgesia in painless parturition and its effect on expression levels of MAPKKER signaling pathway-related molecules in peripheral blood

目的观察针刺辅助镇痛在无痛分娩中的应用及对外周血MAPKERK信号通路相关分子表达水平的影响。方法将我院收治的78例孕产妇采用随机分为对照组(药物镇痛法)与观察组(药物镇痛加针刺辅助镇痛法)各39例,比较两组总产程时间及产后出血量、镇痛效果、外周血MAPKERK信号通路相关分子[信号调节激酶(ERK)、环磷酸腺苷反应元件结合蛋白(CREB)、MAPKK激酶(Raf)表达水平及新生儿阿氏评分(Apgar评分)。结果观察组总产程时间较对照组明显短产后出血量明显低于对照组(P<0.05);观察组疼痛分级明显低于对照组(P<0.05);分娩结束后观察组Raf、ERK、CREB mRNA表达量较对照组明显降低(P<0.05),观察组新生儿出生1min、5min、10min Apgar评分明显高于对照组(P<0.05)。结论针刺辅助镇痛在无痛分娩中的镇痛效果佳,有效促进产程进展、减少产妇出血量并改善新生儿预后,可能与针刺辅助镇痛可有效调节外周血MAPKERK信号通路相关分子表达水平有关。

Objective To observe the application of acupuncture-assisted analgesia in painless parturition and its effect on expression levels of MAPKKER signaling pathway-related molecules in peripheral blood.Methods 78 pregnant and lyingin women were randomly divided into control group(drug analgesia,n=39)and observation group(drug analgesia and acupuncture assisted analgesia,n=39).The total labor time,postpartum bleeding volume,analgesic effects,expression levels of MAPKKER signaling pathway-related molecules[extracellular signal regulated kinase(ERK),cAMP-response element binding protein(CREB),Raf]in peripheral blood and neonatal Apgar score were compared between the two groups.Results The total labor time of observation group was significantly shorter than that of control group,and the postpartum bleeding volume was significantly less than that of control group(P<0.05);the pain grade of observation group was significantly lower than that of control group(P<0.05),the postpartum expression levels of Raf,ERK and CREB in observation group were significantly lower than those in control group(P<0.05),the neonatal Apgar scores after birth at 1 min,5 min and 10 min in observation group were significantly higher than those in control group(P<0.05).Conclusion The analgesic effect of acupunctureassisted analgesia is good in painless parturition.It can effectively promote the progress of labor,reduce the maternal bleeding volume and improve the prognosis of neonates,which may be related to effective regulation of the expression levels of MAPKKER signaling pathway-related molecules in peripheral blood.
