督脉灸联合补肾祛瘀针法治疗强直性脊柱炎急性发作疗效观察

Therapeutic Effect of Du-Moxibustion Combined with Bushen Quyu Acupuncture on Acute Onset of AS

目的:研究督脉灸联合补肾祛瘀针法在强直性脊柱炎急性发作治疗中的临床疗效。方法:研究纳入2017年10月—2019年11月期间湖北省中医院收治的因急性发作入院治疗的强直性脊柱炎患者90例,通过随机数字表法将纳入患者分为两组,观察组和对照组各45例,对照组行补肾祛瘀针法治疗,观察组在对照组基础上加用督脉灸疗法,均连续治疗3周后观察疗效,比较治疗前后的BASDAI、BASFI评分、CRP、ERP水平和血清中疼痛相关介质水平的变化。结果:观察组患者治疗后的总有效率为86.7%(39/45),对照组患者治疗后的总有效率为66.7%(30/45),两组间比较差异有统计学意义(P<0.05),观察组显著优于对照组。观察组患者治疗前BASDAI评分、BASFI评分、CRP水平、ERP水平和PGE2、5-HT、BK、NPY疼痛介质水平与对照组比较,差异无统计学意义(P>0.05),具有临床可比性;上述指标在治疗后,两组组内与治疗前比较均有明显改善(P<0.05);上述指标在治疗后,观察组较对照组改善明显,差异有统计学意义(P<0.05)。结论:督脉灸联合补肾祛瘀针法治疗强直性脊柱炎急性发作具有显著疗效,能够明显改善患者的临床症状,并可明显改善疼痛介质水平。

Objective:To study the therapeutic effect of Du-moxibustion combined with Bushen Quyu acupuncture on acute onset of ankylosing spondylitis(AS).Methods:A total of 90 patients with acute onset of AS were randomly divided into the observation group(n=45)and the control group(n=45).The control group was treated with Bushen Quyu acupuncture;on which basis,the observation group was also treated with Du-moxibustion.The curative effect was observed after 3 weeks of continuous treatment,and the scores of BASDAI and BASFI,as well as the serum levels of CRP,ERP and pain-related mediators were compared before and after the treatment in the two groups.Results:The total effective rate of the observation group was 86.7%(39/45),which was significantly higher than 66.7%(30/45)in the control group(P<0.05).There were no statistically significant differences in the scores of BASDAI and BASFI,as well as in the levels of CRP,ERP and the pain-related mediator of PGE2,5-HT,BK and NPY between the two groups before the treatment(P>0.05);which were all significantly improved after the treatment in the two groups(P<0.05);the improvements in the observation group were more significant(P<0.05).Conclusion:Du-moxibustion combined with Bushen Quyu acupuncture has a significant effect on acute onset of AS,which can significantly improve the clinical symptoms of the patients and reduce the levels of pain mediators.
