足月胎膜早破危险因素及妊娠结局分析

Analysis on risk factors and pregnancy outcome of term premature rupture of membranes

目的探讨足月胎膜早破(TPROM)的危险因素及其对母婴妊娠结局的影响。方法选取240例TPROM产妇作为观察组,另随机选取240例未发生TPROM的产妇为对照组,筛选影响TPROM发生的相关危险因素,比较两组母婴妊娠结局分布的差异性。结果影响TPROM发生的危险因素包括引产流产史(OR=2.392)、生殖道感染(OR=4.459)、妊娠期高血压疾病(OR=3.031)、瘢痕子宫(OR=2.849)、胎位异常(OR=3.626)、腹压增加(OR=5.238)等;观察组产妇产后出血、产褥感染、胎儿窘迫、新生儿窒息、新生儿肺炎等发生率均高于对照组产妇,差异有统计学意义(P<0.05)。结论 TPROM的发生与引产流产史、生殖道感染、妊娠期高血压疾病、瘢痕子宫、胎位异常、腹压增加等密切相关,而且TPROM还会引起不良妊娠结局发生率升高,影响母婴健康和安全。

Objective To explore the risk factors of term premature rupture of membranes (TPROM) and the impact on pregnancy outcome. Methods A total of 240 women with TPROM were selected as observation group,240 women without TPROM were randomly selected as control group. The related risk factors of TPROM were screened,and the pregnancy outcomes in the two groups were compared. Results The risk factors of TPROM included history of induced abortion (OR = 2. 392),genital tract infection (OR = 4. 459),hypertensive disorder complicating pregnancy (OR = 3. 031),scarred uterus (OR = 2. 849),abnormal fetal position (OR = 3. 626),and increased abdominal pressure (OR = 5. 238). The incidence rates of postpartum hemorrhage,puerperal infection,fetal distress,neonatal asphyxia,and neonatal pneumonia in observation group were statistically significantly higher than those in control group (P<0. 05). Conclusion The occurrence of TPROM is closely correlated with history of induced abortion,genital tract infection,hypertensive disorder complicating pregnancy,scarred uterus,abnormal fetal position,and increased abdominal pressure. TPROM also leads to increased incidence rate of adverse pregnancy outcomes and affects health and safety of mothers and infants.
