骨性Ⅲ类下颌偏斜成人上颌牙列垂直特征的研究 被引量:1

Study on the vertical characteristics of the maxillary dentition of skeletal Class Ⅲ mandibular deviation adults

摘要 目的 通过锥体束CT(cone-beam computed tomography,CBCT)研究骨性Ⅲ类下颌偏斜者双侧上颌牙列垂直向特征及其与下颌偏斜间的关系.方法 选取成年骨性Ⅲ类下颌偏斜患者21例(男10例,女11例),测量有关下颌骨偏斜和上颌牙列垂直向测量指标,双侧进行配对t检验或配对秩和检验,下颌偏斜指标与上颌牙列垂直特征差异进行双变量相关分析.结果 两侧Bonwill三角侧边长度,前牙(牙合)平面(AOP)、上颌(牙合)平面(OP)、U4、U5、U6、U7垂直高度差异有统计学意义(P<0.05),颏部偏向侧(D侧)-颏部偏离侧(C侧)的差值分别为(-3.46±3.67)mm、4.01°±5.70°、1.40°±3.21°、(-1.73±2.37) mm、(-1.70±2.31)mm、(-1.90±2.29)mm、(-2.22±2.69) mm;下颌偏斜程度与双侧后牙连线倾斜角度、AOP差异、OP差异呈正相关(P<0.05).结论 骨性Ⅲ类下颌偏斜患者存在两侧后牙垂直高度不一致. Objective The purpose of this study was to analyze vertical characteristics of bilateral maxillary teeth using cone-beam computed tomography(CBCT)and evaluate the correlations between vertical characteristics and skeletal mandibular deviation variables in patients with skeletal Class I malocclusion and mandibular deviation,Methods Twenty-one patients(10 male adults,11 female adults)with skeletal Class I malocclusion and mandibular deviation were included.Skeletal deviation and dental vertical measurements were acquired.All measurements were compared between the deviated and contralateral sides by paired t tests or the Wilcoxon signed-rank test.The correlations between vertical characteristics of maxillary teeth and skeletal mandibular deviation variables were calculated by bivariate correlation analysis.Results There were statistical difference in Bonwill triangle side length,anterior occlusal plane,maxillary occlusal plane and vertical height of maxillary posterior teeth between the deviated and contralateral sides(P<0.05).D side minus C side difference are(-3.46±3.67)mm,4.01±5.70°,1.40°±3.21°,(-1.73±2.37)mm,(-1.70±2.31)mm,(-1.90±2.29)mm and(-2.22±2.69)mm respectively;The degree of mandibular deviation is positively correlated with the inclination angle of bilateral posterior teeth,AOP difference,and OP difference(P<<0.05).Conclusions The vertical height of two posterior teeth was not consistent in patients with skeletal Class I malocclusion and mandibular deviation.
