CHIVA手术与传统手术治疗大隐静脉曲张疗效比较

A Comparative Study of CHIVA and Traditional Operation in the Treatment of Varicose Great Saphenous Vein

目的探讨静脉功能不全的保守血流动力学管理(ambulatory conservative hemodynamic management of varicose veins,CHIVA)(CHIVA为法文缩写)治疗大隐静脉曲张的安全性、可行性、可靠性。方法将我科从2016年1月至2018年1月收治大隐静脉曲张病人100例,随机分为两组,分别为CHIVA组和传统组,每组50例。CHIVA组行CHIVA手术,传统组行大隐静脉高位结扎及抽剥术。比较两组手术时间、术中出血量、住院时间、术后并发症及随访2年的复发率。结果CHIVA组在手术时间、术中出血量、住院天数、术后并发症等方面优于传统手术组(P<0.05),术后随访2年的复发率与传统手术无差异(P>0.05)。结论CHIVA技术治疗大隐静脉曲张是安全的、可行的、可靠的,比传统手术更经济、更微创,值得推广。

Objective To evaluate the efficacy,feasibility and reliability of ambulatory conservative hemodynamic management of varicose veins(CHIVA)for varicose great saphenous vein.Methods From January 2016 to January 2018,100 cases with varicose of great saphenous vein were randomly divided into two groups:CHIVA group and the traditional operation group,with 50 cases for each.CHIVA group underwent CHIVA operation,while the traditional group underwent high ligation and stripping of great saphenous vein.Operation time,bleeding volume,hospitalization time,postoperative complications and the recurrence rate of 2 years'follow-up were compared between the two groups.Results CHIVA group was superior to the traditional operation group in operation time,amount of bleeding,hospitalization time,postoperative complications(P<0.05).The recurrence rate after 2 years'follow-up was indifferent(P>0.05).Conclusion CHIVA technique is safe,feasible and reliable in the treatment of varicocele of saphenous vein.It is more economical and minimally invasive than tradional operation,and is worth popularizing.
