对保乳术与改良根治术在早期乳腺癌患者治疗中的应用方式和效果分析

Analysis of the Application Mode and Effect of Breast Conserving Surgery and Modified Radical Mastectomy in the Treatment of Patients with Early Breast Cancer

目的对早期乳腺癌患者采用保乳术与改良根治术治疗的应用效果进行探讨。方法选取无锡市第二人民医院2014年5月至2017年5月收治的160例早期乳腺癌患者进行研究,根据手术方式的不同,将患者分成两组,其中改良组80例采用改良根治术,保乳组80例采用保乳术,对比两组患者的手术指标、术后近远期复发率、转移率、死亡率及并发症发生率。结果保乳组手术用时短于改良组,住院时间短于改良组,术中出血量少于改良组,差异有统计学意义(P<0.05);保乳组与改良组术后1年及3年复发率、转移率及死亡率对比,差异无统计学意义(P>0.05);保乳组并发症率为7.50%,低于改良组的25.00%,差异有统计学意义(P<0.05)。结论早期乳腺癌患者,采用保乳术及改良根治术治疗的术后近远期复发率、转移率及死亡率相当,但前者手术耗时短、手术出血量少、并发症少,患者恢复更快。

Objective To investigate the efficacy of breast conserving surgery and modified radical surgery in the treatment of early breast cancer.Methods Selection in Wuxi Second People's Hospital from May 2014 to May 2017 treated 160 cases of patients with early breast cancer,according to the different methods of operation,the patients was splited into two groups,80 cases in the modified group were treated with modified radical operation,and 80 cases in the breast conserving group were treated with breast conserving operation.The operative parameters,postoperative recurrence rate,metastasis rate,mortality rate and complication rate were compared between the two groups.Results The operation time of the breast conserving group was shorter than that of the modified group,the time of hospital stay was all shorter than that of the modified group,and the intraoperative blood loss was less than that of the modified group,and the difference was statistically significant(P<0.05).There was no significant difference in recurrence rate,metastasis rate and mortality rate between the breast conserving group and the modified group at 1 and 3 years after operation(P>0.05).The complication rate of breast conserving group was 7.50%,which was lower than 25.00%of the modified group,and the difference was statistically significant(P<0.05).Conclusion For the patients with early breast cancer,the postoperative near and long term recurrence rate,metastasis rate and mortality rate are similar by breast conserving surgery and modified radical surgery.But the patients of breast conserving group's operation time is shorter,the amount of bleeding and complications are less,and the patients recover faster.
