血培养联合超敏C反应蛋白在新生儿败血症早期诊断中的价值体会 被引量:1

摘要 目的探讨血培养联合超敏C反应蛋白在新生儿败血症早期诊断中的价值。方法纳入的研究对象为盐城市妇幼保健院2018年8月至2020年8月收治的100例新生儿败血症患儿,将其设为观察组,并选取同期健康的新生儿100例作为对照组,采用免疫散射比浊法检测患儿的超敏C反应蛋白水平,采用BACTEC9120血培养仪对患儿进行血培养,采用全自动细菌鉴定仪对细菌进行鉴定,记录观察组血培养结果,对比两组血清超敏C反应蛋白水平、超敏C反应蛋白水平阳性检测结果及检测结果的灵敏度、准确度及特异度。结果100份血标本中,检出革兰阴性菌30株(30.00%),检出革兰阳性菌67株(67.00%),真菌3株(3.00%);观察组超敏C反应蛋白水平显著高于对照组(P<0.05);观察组超敏C反应蛋白水平阳性率80.00%显著高于对照组40.00%(P<0.05);血培养+超敏C反应蛋白检测结果的灵敏度、准确度及特异度高于单一血培养及超敏C反应蛋白检测结果(P<0.05)。结论超敏C反应蛋白可作为新生儿败血症诊断的重要参考指标,联合血培养可鉴别患儿病原体种类,提高诊断结果的灵敏度及特异度,为疾病的后续治疗提供重要的参考价值。 Objective To investigate the value of blood culture combined with high sensitivity C-reactive protein in the early diagnosis of neonatal sepsis.Methods The research object was 100 cases of children with neonatal sepsis in Yancheng Maternal and Child Health Hospital from August 2018 to August 2020.They were included in the observation group,100 cases with neonatal health and the same period as the control group,children with immune scattering turbidimetric method is used to detect the hypersensitive C-reactive protein level,using BACTEC9120 blood culture instrument for children with blood culture,adopt automatic bacteria identification instrument for bacteria identification,record group blood cultures as a result,The level of serum hypersensitive C-reactive protein,the positive test results of hypersensitive C-reactive protein and the sensitivity,accuracy and specificity of the test results were compared between the two groups.Results Among the 100 blood samples,30 strains of Gram-negative bacteria(30.00%),67 strains of Gram-positive bacteria(67.00%)and 3 strains of fungi(3.00%)were detected.The level of hypersensitive C-reactive protein in observation group was significantly higher than that in control group(P<0.05);The positive rate of hypersensitive C-reactive protein(80.00%)in observation group was significantly higher than that in control group(40.00%,P<0.05).The sensitivity,accuracy and specificity of blood culture+hypersensitive C-reactive protein were higher than those of single blood culture and hypersensitive C-reactive protein(P<0.05).Conclusion Hypersensitive C-reactive protein can be used as an important reference index for the diagnosis of neonatal sepsis.Combined with blood culture,it can identify the types of pathogens in children,improve the sensitivity and specificity of the diagnostic results,and provide important reference value for the follow-up treatment of the disease.
