中医针刺结合产后康复治疗仪对初产妇产后乳汁淤积的影响

Effects of traditional Chinese medicine acupuncture combined with postpartum rehabilitation treatment apparatus on postpartum galactostasis in primiparas

目的探讨中医针刺结合产后康复治疗仪对初产妇产后乳汁淤积的影响。方法选取2018年6月至2020年6月西北妇女儿童医院康复中心收治的115例产后乳汁淤积的初产妇作为研究对象,按随机数表法分为观察组57例和对照组58例。对照组初产妇使用产后康复治疗仪联合乳房按摩治疗,观察组初产妇使用产后康复治疗仪联合中医针刺治疗,均治疗5 d,比较两组初产妇的治疗效果、临床症状评分、乳房硬结消失时间和泌乳开始时间、乳汁排出通畅度以及1个月内复发率。结果治疗期间,观察组初产妇泌乳开始时间、乳房硬结消失时间分别为(1.75±0.62)d、(2.37±0.59)d,明显快于对照组的(2.37±0.59)d、(3.67±0.72)d,差异有统计学意义(P<0.05);疗程结束后,观察组初产妇的治疗有效率为100.00%,明显高于对照组的93.10%,差异有统计学意义(P<0.05);治疗后,观察组初产妇的乳房疼痛、肿块大小和泌乳情况评分分别为(0.65±0.21)分、(0.48±0.19)分、(0.74±0.22)分,明显低于对照组的(1.27±0.32)分、(0.65±0.16)分、(1.16±0.26)分,差异均有统计学意义(P<0.05);观察组初产妇的乳汁排出困难率为0,明显低于对照组的10.34%,差异有统计学意义(P<0.05);随访1个月,观察组和对照组初产妇的乳汁淤积复发率比较差异无统计学意义(P>0.05)。结论中医针刺结合产后康复治疗仪可缩短乳汁淤积的初产妇泌乳开始时间及乳房硬结消失时间,改善乳汁排出通畅度,降低乳房疼痛,疗效显著。

Objective To explore the effects of traditional Chinese medicine(TCM)acupuncture combined with postpartum rehabilitation treatment apparatus on postpartum galactostasis in primiparas.Methods A total of 115 primiparas with postpartum galactostasis admitted to Northwest Women and Children's Hospital from June 2018 to June 2020 were enrolled and divided into observation group(57 cases,postpartum rehabilitation treatment apparatus combined with TCM acupuncture)and control group(58 cases,postpartum rehabilitation treatment apparatus combined with breast massage)by random number table method.All were treated for 5 d.The curative effect,scores of clinical signs,disappearance time of breast induration,start time of lactating,patency of lactic discharge,and recurrence rate within 1 month were compared between the two groups.Results During treatment,start time of lactating and disappearance time of breast induration in observation group were(1.75±0.62)d and(2.37±0.59)d,significantly shorter than(2.37±0.59)d and(3.67±0.72)d in the control group(P<0.05).At the end of treatment course,the response rate of treatment in observation group was higher than that in control group(100.00%vs 93.10%,P<0.05).After treatment,scores of breast pain,mass size,and lactation in the observation group were(0.65±0.21)points,(0.48±0.19)points,and(0.74±0.22)points,significantly lower than(1.27±0.32)points,(0.65±0.16)points,(1.16±0.26)points in the control group(P<0.05).The difficulty rate of lactic discharge in the observation group was significantly lower than that in control group(0 vs 10.34%,P<0.05).The two groups were followed up for 1 month.There was no significant difference in recurrence rate of galactostasis between the observation group and control group(P>0.05).Conclusion TCM acupuncture combined with postpartum rehabilitation treatment apparatus can shorten start time of lactating and disappearance time of breast induration in primiparas with postpartum galactostasis,improve patency of lactic discharge,and reduce breast pain,with significant curative effect.
