摘要
目的探讨胃癌患者血清RDW与NLR联合检测在胃癌诊断中的价值。方法选取2018年8月~2020年8月我院行胃镜检查的365例患者作为研究对象,根据胃镜及病理结果分为胃癌组180例和胃部良性疾病组185例,比较两组血清RDW、NLR、CEA及CA724水平及阳性检出率,采用ROC曲线分析各指标单独及联合检测对胃癌的诊断效能。结果胃癌组血清RDW、NLR、CEA、CA724水平及阳性检出率均高于胃部良性疾病组,差异有统计学意义(P<0.05)。ROC曲线分析显示,RDW、NLR单独检诊断胃癌的灵敏度、特异度、约登指数、最佳cut-off值高于CEA、CA724单独诊断(P<0.05);RDW和NLR联合诊断的灵敏度及特异度高于CEA和CA724联合检测(P<0.05)。结论胃癌患者血清RDW、NLR水平较胃部良性疾病患者增高,血清RDW和NLR联合可作为胃癌辅助诊断的血清标志物,可为患者是否需要进一步行胃镜检查提供重要参考。
Objective To explore the value of combined detection of serum RDW and NLR in the diagnosis of gastric cancer.Methods A total of 365 patients who underwent gastroscopy in our hospital from August 2018 to August 2020 were selected as the research objects.According to the results of gastroscopy and pathology,they were divided into 180 cases of gastric cancer group and 185 cases of gastric benign disease group.The serum RDW,NLR,CEA and CA724 levels and positive detection rate of the two groups were compared.The ROC curve was used to analyze the diagnostic efficacy of individual and combined detection of each index for gastric cancer.Results The levels of serum RDW,NLR,CEA,CA724 and the positive detection rate in the gastric cancer group were higher than those in the gastric benign disease group,the difference was statistically significant(P<0.05).ROC curve analysis showed that the sensitivity,specificity,Youden index,and best cut-off value of RDW and NLR alone in the diagnosis of gastric cancer were higher than CEA and CA724 alone(P<0.05);The sensitivity and specificity of combined diagnosis of RDW and NLR were higher than combined detection of CEA and CA724(P<0.05).Conclusion The levels of serum RDW and NLR in patients with gastric cancer are significantly higher than those in patients with benign gastric diseases.The combination of serum RDW and NLR can be used as a serum marker for the auxiliary diagnosis of gastric cancer,which can provide an important reference for whether patients need further gastroscopy.
