T2DM患者血清25-OH-VD表达水平与糖尿病肾病发生的相关性

Correlation between the expression level of serum 25-OH-VD and occurrence of diabetic nephropathy in patients with T2DM

目的探讨2型糖尿病(T2DM)患者血清25羟基维生素D(25-OH-VD)表达水平与糖尿病肾病发生的相关性。方法选择2020年1—10月本院收治的120例T2DM患者作为研究对象,根据是否发生糖尿病肾病分为发生糖尿病肾病组(41例)和未发生糖尿病肾病组(79例)两组。两组均检测血清25-OH-VD水平、尿白蛋白排泄率(UAE),分析25-OH-VD表达水平与UAE、糖尿病肾病发生的相关性。结果发生糖尿病肾病组的血清25-OH-VD水平低于未发生糖尿病肾病组,发生糖尿病肾病组的UAE高于未发生糖尿病肾病组,差异有统计学意义(P<0.05);经双变量Pearson直线相关性检验,血清25-OH-VD水平与UAE呈负相关(r<0,P<0.05)。经Logistic回归分析显示,血清25-OH-VD低表达是T2DM患者发生糖尿病肾病的影响因素(OR>1,P<0.05)。结论血清25-OH-VD表达水平、UAE水平与糖尿病肾病的发生密切相关,血清25-OH-VD低表达预示T2DM患者发生糖尿病肾病风险较高,临床可通过检测25-OH-VD水平来预防糖尿病肾病的发生。

Objective To investigate the relationship between the expression of serum 25-OH-VD(25-OH-VD)and the occurrence of diabetic nephropathy in patients with type 2 diabetes(T2DM).Methods 120 T2DM patients treated in our hospital from January 2020 to October 2020 were selected as study subjects.According to whether diabetic nephropathy occurred,they were divided into the diabetic nephropathy group(41 cases)and the non-diabetic nephropathy group(79 cases).Serum levels of 25-OH-VD and urine albumin excretion rate(UAE)of the two groups were tested.The correlation between the 25-OH-VD expression level and UAE,and the occurrence of diabetic nephropathy were analyzed.Results The serum 25-OH-VD level of the patients in diabetic nephropathy group was lower than that of the non-diabetic nephropathy group,and the UAE of the diabetic nephropathy group was higher than that of the non-diabetic nephropathy group,the difference was statistically significant(P<0.05).According to the examination of bivariate Pearson linear correlation,the serum 25-OH-VD level was negatively correlated with UAE(r<0,P<0.05).Logistic regression analysis showed that low serum 25-OH-VD expression was an influencing factor for diabetic nephropathy in T2DM patients(OR>1,P<0.05).Conclusions Serum 25-OH-VD expression level and UAE level are closely related to the occurrence of diabetic nephropathy.The low expression of serum 25-OH-VD indicates that patients with T2DM have a higher risk of diabetic nephropathy.Clinically,the occurrence of diabetic nephropathy could be prevented by detecting the level of 25-OH-VD.
