摘要
目的探究妇科超声影像报告和数据系统(GI-RADS)联合三维超声造影(3D-CEUS)在卵巢良恶性病变鉴别诊断中的应用价值。方法选取2018年8月-2020年8月在本院就诊治疗的卵巢肿块患者152例作为研究对象,对其进行二维超声和3D-CEUS,将所得的二维超声图像进行GI-RADS评估,根据GI-RADS与3D-CEUS的恶性指征对患者进行良恶性病变鉴别诊断,后与手术病理结果相比较,分析GI-RADS联合3D-CEUS鉴别诊断卵巢良恶性病变的价值。结果经手术探查152例患者中良性肿瘤98例,恶性肿瘤54例;GI-RADS与3D-CEUS的恶性指征标准均具有鉴别诊断卵巢良恶性病变的意义(P<0.05);GI-RADS与3D-CEUS的恶性指征标准还具有较高的卵巢恶性肿瘤诊断价值;GI-RADS联合3D-CEUS的诊断灵敏度、特异度、阳性预测值、阴性预测值均要高于单一GI-RADS诊断(P<0.05)。结论 GI-RADS联合3D-CEUS可提高卵巢良恶性病变鉴别准确率。
Objective To explore the application value of the gynecology imaging reporting and data system(GI-RADS)combined with three-dimensional contrast-enhanced ultrasound(3D-CEUS)in the differential diagnosis of benign and malignant ovarian lesions.Methods Between August 2018 and August 2020,152 patients with ovarian lumps those admitted to our hospital were selected as study subjects and subjected to two-dimensional ultrasound and 3D-CEUS examination.The two-dimensional ultrasound images obtained were evaluated by GI-RADS,and diagnostic results of GI-RADS and 3D-CEUS were compared with the surgical pathological results to analyze the value of GI-RADS combined with 3D-CEUS in the differential diagnosis of benign and malignant ovarian lesions.Results Surgical exploration found 98 cases with benign tumors and 54 cases with malignant tumors among the 152 patients.Both GI-RADS and 3D-CEUS had high value in the differential diagnosis of benign and malignant ovarian lesions as well as diagnosis of malignant ovarian lesions(P<0.05).Besides,combined diagnosis of the two method had higher sensitivity,specificity,positive predictive value,and negative predictive value than GI-RADS method alone(P<0.05).Conclusions GI-RADS combined with 3D-CEUS could improve the differential diagnosis accuracy for benign and malignant ovarian lesions.
