全过程优质护理在老年胆结石并急性胆囊炎患者围术期的实践研究

Study on the practice of the whole process of high quality nursing in perioperative period of the elderly patients with gallstone complicated with acute cholecystitis

目的老年胆结石并急性胆囊炎患者围术期中应用全过程优质护理的实践作用观察。方法选择2017年7月-2018年12月本院收治的实施手术治疗的老年胆结石并急性胆囊炎患者120例作为研究对象,随机数表法分为观察组和对照组两组,每组各60例。对照组实施常规的围术期护理,观察组实施常规围术期护理+全过程优质护理,比较两组护理前后的心理状态,对比两组术后的恢复指标和术后1周内的并发症发生率。结果护理后两组的Beck焦虑量表(BAI)和Beck抑郁量表(BDI)评分均下降(P<0.05),与对照组比较观察组更低(P<0.05),且观察组BAI、BDI评分降低量均高于对照组(P<0.05);观察组术后首次下床时间和住院时间均短于对照组(P<0.05);两组术后1周内的并发症发生率比较观察组更低(P<0.05)。结论在老年胆结石并急性胆囊炎患者围术期应用全过程优质护理对于改善患者的负性情绪,促进患者的恢复,减少并发症有显著的作用。

Objective To study the practice effect of applying whole process of high-quality nursing in perioperative perioperative of the elderly patients with gallstone complicated with acute cholecystitis.Methods 120 elderly patients with gallstone complicated with acute cholecystitis those who underwent surgical treatment in our hospital from July 2017 to December 2018 were selected as study subjects and divided into observation group and control group according random number table method,60 cases in each group.Patients in control group were given routine perioperative nursing and routine perioperative nursing plus whole-process high-quality nursing was given to the patients in observation group.The psychological state before and after nursing was compared between the two groups,and the recovery indexes after operation and the incidence of complications within the next week after operation were compared between the two groups.Results After nursing,the scores of Beck Anxiety Inventory(BAI)and Beck Depression Inventory(BDI)decreased in both groups(P<0.05),which were lower in the observation group than in the control group(P<0.05).The decrease values of BAI and BDI scores in the observation group were higher than those in the control group(P<0.05).The first time out of bed and hospitalization time in the observation group were shorter than those in the control group(P<0.05).Compared with the incidences of complications in the two groups within one week after operation,the observation group was lower(P<0.05).Conclusions The application of quality nursing in the perioperative period of elderly patients with gallstones and acute cholecystitis can improve their negative emotions,promote their recovery and reduce complications.
