CA21-1和SCC检测在非小细胞肺癌的诊断及预后评定中的价值分析 被引量:4

摘要 目的探讨细胞角蛋白19片段抗原(CA21-1)和鳞状细胞癌相关抗原(SCC)检测在非小细胞肺癌诊断与预后评定中的价值。方法选择经病理证实的非小细胞肺癌患者78例作为肺癌组,同期选择健康人群78例作为健康组。检测两组血清CA21-1和SCC含量,调查反映患者的预后指标-淋巴结转移情况并进行相关性分析。结果肺癌组的血清CA21-1和SCC含量显著高于健康组(P<0.05)。ROC曲线分析显示:CA21-1和SCC诊断非小细胞肺癌的曲线下面积分别0.765,0.675,0.714,诊断敏感性与特异性都大于85.0%。78例肺癌中发生淋巴结转移26例,Pearson相关分析显示:CA21-1和SCC与淋巴结转移存在相关性(P<0.05);二分类变量Logistic回归分析显示:CA21-1和SCC为影响肺癌患者淋巴结转移的主要影响因素(P<0.05)。结论CA21-1和SCC在非小细胞肺癌患者中呈现高表达状况,与患者的淋巴结转移存在相关性,也是导致淋巴结转移发生的主要影响因素。 Objective To investigate the value of Cytokeratin 19 Fragment Antigen(CA21-1)and squamous cell carcinoma associated antigen(SCC)in the diagnosis and prognosis assessment of non-small cell lung cancer.Methods 78 patients with non-small cell lung cancer confirmed by pathology were selected as the lung cancer group,and 78 healthy patients were selected as the healthy group at the same time.The serum levels of CA21-1 and SCC were detected in the two groups,and the prognostic indicators reflected the patient's lymph node metastasis were investigated and were given correlation analysis.Results The serum levels of CA21-1 and SCC in the lung cancer group were significantly higher than those in the healthy group(P<0.05).ROC curve analysis showed that the area under the curve of CA21-1 and SCC in the diagnosis of non-small cell lung cancer were 0.765 and 0.714,respectively,and the diagnostic sensitivity and specificity were all more than 85.0%.There were 26 cases of lymph node metastasis in 78 cases of lung cancer.Pearson correlation analysis showed that CA21-1 and SCC were correlated with lymph node metastasis(P<0.05);the logistic regression analysis of binary variables showed that CA21-1 and SCC were the main influencing factors of patients with lymph node metastasis(P<0.05).Conclusion CA21-1 and SCC are highly expressed in patients with non-small cell lung cancer,which are related to the lymph node metastasis of patients,and are also the main influencing factors for the occurrence of lymph node metastasis.
