轮廓化鼻内镜手术与功能性鼻内镜手术治疗难治性鼻-鼻窦炎患者的效果比较

Comparison of effects of radical sinus surgery and functional endoscopic sinus surgery in treatment of patients with refractory rhinosinusitis

目的:比较轮廓化鼻内镜手术(RSS)与功能性鼻内镜手术(FESS)治疗难治性鼻-鼻窦炎(DTRS)患者的效果。方法:选取80例DTRS患者为研究对象,按照随机数字表法分为对照组(n=40)与观察组(n=40)。对照组采用FESS治疗,观察组采用RSS治疗,比较两组临床疗效,手术前后流涕、头痛、嗅觉障碍症状的视觉模拟评分法(VAS)评分,鼻内镜Lund-Kennedy评分以及并发症发生率。结果:观察组治疗总有效率为97.50%,高于对照组的80.00%,差异有统计学意义(P<0.05);术后6个月,两组各症状VAS评分及鼻内镜Lund-Kennedy评分均降低,且观察组低于对照组,差异有统计学意义(P<0.05);两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论:RSS治疗DTRS患者的效果优于FESS,可有效改善患者的临床症状及内镜评分,安全性与FESS相当。

Objective:To compare effects of radical sinus surgery(RSS)and functional endoscopic sinus surgery(FESS)in treatment of patients with refractory rhinosinusitis(DTRS).Methods:80 DTRS patients were selected as the research objects,and were divided into control group(n=40)and observation group(n=40)according to the random number table method.The control group was treated with FESS,while the observation group was treated with RSS.The clinical effects,the visual analogue scale(VAS)scores of the symptoms such as runny nose,headache,and olfactory disorders before and after the surgery,the Lund-Kennedy score of nasal endoscopy and the incidence of complications were compared between the two groups.Results:The total effective rate of treatment in the observation group was 97.50%,which was higher than 80.00%in the control group,and the difference was statistically significant(P<0.05).6 months after the surgery;those of the observation group were lower than those of the control group;and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusions:RSS is more effective in the treatment of DTRS than FESS.It can effectively improve the clinical symptoms and the endoscopic score with a safety equivalent to FESS.
