摘要
调查分析丽水地区健康体检人群高尿酸血症发病情况及相关危险因素,以期降低本地区居民痛风发病率。本研究随机抽取2018年1月—2020年12月期间在丽水市中心医院健康体检的15653例血液样本数据,回顾性分析该人群高尿酸血症的发病率,运用多因素Logistic回归分析高尿酸血症发病率和性别、年龄、血压、BMI、血脂、肌酐、尿素氮、血糖及高低密度脂蛋白等因素的相关性。结果显示:在入组的15653例体检者中,血尿酸平均值为(328.15±69.6)μmol/L,其中男性血尿酸平均值为(378.72±82.32)μmol/L,女性血尿酸平均值为(285.75±62.79)μmol/L,差异有统计学意义(P<0.05);总体人群高尿酸血症发病率23.33%(3652/15653),男性高尿酸血症发病率30.42%(2571/8451),女性高尿酸血症发病率15.01%(1081/7202),差异有统计学意义(χ^(2)=516.379,P<0.05)。从年龄方面统计,男性40岁之前发病率高,为35.97%(741/2060);女性在50周岁以上高尿酸发生率逐年升高,各组间差异有统计学意义。多因素Logistic回归分析显示,男性、肥胖、高血压、高甘油三酯是高尿酸血症发生的独立危险因素(OR值分别为0.345、1.615、1.312、2.122,P均<0.01)。研究表明:丽水地区高尿酸血症发病率高,且男性高于女性,男性发病年龄趋于年轻化,应引起临床医师重视。丽水地区男性、肥胖、高血压、高甘油三酯人群血尿酸明显高于正常人群,因此,以上4因素可视为该地区高尿酸血症的危险因素。
To investigate the incidence of hyperuricemia and related risk factors in the health checkup population in this area,in order to reduce the incidence of gout in this area.This study randomized 15653 blood samples from the health checkups in our hospital from January 2018-December 2020,and retrospec-tively analyzed the incidence of hyperuricemia in this population.Multivariate logistic regression analysis was used to analyze hyperuricemia.Incidence and gender age,blood pressure,BMI,blood lipids,creatinine,urea nitrogen,blood sugar and high and low density lipoprotein and other factors.The results reveal:The average value of blood uric acid was(328.15±69.6)μmol/L among the 15653 medical examinations,and the mean value of male uric acid was(378.72±82.32)μmol/L.There was no significant difference in serum uric acid between(285.75±62.79)μmol/L(P<0.05);the incidence of hyperuricemia in the general population was 23.33%(3652/15653),and the incidence of hyperuricemia in males was 30.42%(2571/8451),the incidence of hyperuricemia in women was 15.01%(1081/7202),the difference was statistically significant(χ^(2)=516.379,P<0.05).From the age aspect,the incidence rate of males before the age of 40 was35.97%(741/2060);the incidence of high uric acid in women over 50 years old increased year by year,the difference between the groups was statistically significant.Multivariate logistic regression analysis showed that male,obese,high Blood pressure and high triglycerides were independent risk factors for hyperuricemia(OR values were 0.345、1.615、1.312、2.122,P<0.01).The incidence of hyperuricemia in this area is high,and male is higher than female.The age of onset of male tends to be younger,which should be paid attention to by clinicians.In the region,males,obesity,hypertension,and high triglyceride populations have significantly higher blood uric acid than normal people,which may be considered a risk factor for hyperuricemia in the region.
