自身免疫性肝炎患者外周血中Linc00324的表达及临床意义

Expression and clinical significance of Linc00324 in peripheral blood of patients with autoimmune hepatitis

目的:探讨自身免疫性肝炎患者外周血血清中Linc00324的表达及临床意义。方法:选取2016年9月至2019年6月经临床确诊的自身免疫性肝炎患者(自身免疫性肝炎组)55例及体检健康者(正常组)61例,收集外周血标本并分离血清,通过实时荧光定量聚合酶链式反应(qRT-PCR)检测受试者血清Linc00324表达水平,通过流式细胞术检测受试者外周血Th、Ts细胞比例;观察两组受试者临床指标,包括性别、年龄、谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBil)、碱性磷酸酶(ALP)、谷氨酰转肽酶(GGT)、IgG、抗核抗体(ANA)、抗平滑肌抗体(SMA)、抗线粒体抗体(ASA),并通过Pearson法分析自身免疫性肝炎患者血清Linc00324水平与临床指标的相关性,应用受试者工作特征曲线(ROC)分析血清Linc00324水平对自身免疫性肝炎的诊断作用。结果:与正常组相比,自身免疫性肝炎组患者血清Linc00324、IgG、ALT、AST、TBil、ALP、GGT水平、ANA阳性率、SMA阳性率及Th细胞比例、Th/Ts比值显著升高(P<0.05),Ts细胞比例显著降低(P<0.05);自身免疫性肝炎患者血清Linc00324水平与ALT、AST、ALP、IgG、Th、Th/Ts比值正相关(P<0.05),与Ts比例负相关(P<0.05);ROC结果显示,血清Linc00324水平诊断自身免疫性肝炎曲线下面积为0.795,敏感度为78.2%,特异度为81.3%。结论:自身免疫性肝炎患者血清Linc00324高表达,可能是自身免疫性肝炎的诊断指标。

Objective:To investigate the expression and clinical significance of Linc00324 in peripheral blood serum of patients with autoimmune hepatitis.Methods:From September 2016 to June 2019,55 patients with clinical diagnosed autoimmune hepatitis and 61 healthy persons were selected,peripheral blood samples were collected and serum was separated,the expression level of Linc00324 was detected by real-time fluorescence quantitative PCR(qRT-PCR),the proportions of Th and Ts cells in the peripheral blood were detected by flow cytometry,the clinical indexes of the subjects were measured,including sex,age,alanine aminotransferase(ALT),transglutaminase(AST),total bilirubin(TBIL),alkaline phosphatase(ALP),glutamyltranspeptidase(GGT),IgG,anti nuclear antibody(ANA),anti smooth muscle antibody(SMA),anti mitochondrial antibody(ASA),the correlation between serum Linc00324 level and clinical indexes were analyzed by Pearson method,and the diagnostic effect of serum Linc00324 level on autoimmune hepatitis was analyzed by receiver operating characteristic curve(ROC).Results:Compared with the normal group,the levels of Linc00324,IgG,ALT,AST,TBIL,ALP,GGT in serum,ANA positive rate,SMA positive rate,Th cell proportion and Th/TS level were significantly higher in the autoimmune hepatitis group(P<0.05),while the proportion of Ts cells decreased significantly(P<0.05);the level of Linc00324 was positively correlated with ALT,AST,ALP,IgG,Th,Th/TS(P<0.05),and negatively correlated with Ts proportion(P<0.05);ROC results showed that the area under the curve of serum Linc00324 level in the diagnosis of autoimmune hepatitis was 0.795,the sensitivity was 78.2%,and the specificity was 81.3%.Conclusion:The high expression of linc00324 in serum of patients with autoimmune hepatitis may be a diagnostic index of autoimmune hepatitis.
