干眼症患者泪液IL-1β、TNF-α、TSLP水平与BUT值、FL评分的相关性分析 被引量:18

摘要 目的探讨干眼症患者泪液白细胞介素1β(interleukin-1β,IL-1β)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)、胸腺基质淋巴细胞生成素(thymic stromal lymphopoietin,TSLP)水平与泪膜破裂时间(break-up time,BUT)值、角膜荧光素染色(corneal fluorescein staining,FL)评分的相关性。方法选择干眼症患者120例为研究组,同期健康志愿者100例为对照组。采集泪液,检测IL-1β、TNF-α、TSLP水平,比较2组BUT值及FL评分。分析泪液IL-1β、TNF-α、TSLP水平与BUT值、FL评分的相关性。结果研究组泪液IL-1β、TNF-α、TSLP水平均显著高于对照组,差异有统计学意义(P<0.05)。研究组BUT值明显低于对照组,FL评分明显高于对照组,差异有统计学意义(P<0.05)。Pearson相关分析结果显示,泪液IL-1β、TNF-α、TSLP水平与BUT值呈显著负相关(P<0.01),与FL评分呈显著正相关(P<0.01)。ROC曲线显示,泪液IL-1β、TNF-α、TSLP水平对干眼症具有一定诊断价值(AUC=0.830,0.885,0.838,P<0.01),且三项联合(AUC=0.920,P<0.01)诊断价值较高。结论干眼症患者泪液IL-1β、TNF-α、TLSP水平与BUT值、FL评分均显著相关,IL-1β、TNF-α、TLSP水平升高导致泪膜稳定性降低及眼表损伤,联合检测泪液IL-1β、TNF-α、TLSP水平可用于干眼症的辅助诊断。 Objective To investigate the correlation of the levels of interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α)and thymic stromal lymphopoietin(TSLP)in tear fluid of patients with dry eye with tear film break-up time(BUT)value and corneal fluorescein staining(FL)score.Methods A total of 120 patients with dry eye were selected as the research group,and 100 healthy volunteers during the same period were selected as the control group.Tear fluid was collected,IL-1β,TNF-α,TSLP levels were detected,and the BUT value and FL score of the two groups were compared.The correlation of IL-1β,TNF-α,TSLP levels in tear fluid with BUT value and FL score was analyzed.Results The levels of IL-1β,TNF-αand TSLP in tear fluid of the research group were significantly higher than those of the control group,and the difference was statistically significant(P<0.05).The BUT value of the research group was significantly lower than that of the control group,while the FL score was significantly higher than that of the control group,with significant difference(P<0.05).Pearson correlation analysis results showed that IL-1β,TNF-α,TSLP levels in tear fluid were significantly and negatively correlated with BUT values(P<0.01),but significantly and positively correlated with FL scores(P<0.01).The ROC curve revealed that the levels of IL-1β,TNF-α,and TSLP in tear fluid had a certain diagnostic value for dry eye(AUC=0.830,0.885,0.838,P<0.01),and the diagnostic value of the combination of the three was higher(AUC=0.920,P<0.01).Conclusion The levels of IL-1β,TNF-αand TLSP in the tear fluid of patients with dry eye are significantly correlated with the BUT value and FL score.Increased levels of IL-1β,TNF-αand TLSP lead to a decrease in tear film stability and ocular surface damage.Combination detection of the levels of IL-1β,TNF-αand TLSP in tear fluid can be used for the auxiliary diagnosis of dry eye.
