宫腔镜下子宫内膜微刺激术联合子宫内膜浆细胞检查对IVF-ET患者妊娠结局影响

Effects of hysteroscopic endometrial micro stimulation combined with endometrial plasma cell examination on pregnancy outcome in IVF-ET patients

目的探究宫腔镜下子宫内膜微刺激术联合子宫内膜浆细胞检查对体外受精-胚胎移植(IVF-ET)患者妊娠结局的临床效果及预后影响。方法选取2016年4月至2018年3月在我院收治的60例行IVF-ET患者,根据其在下一周期前是否行宫腔镜及内膜浆细胞检查分为宫腔镜检查组(实验组)和未行宫腔镜检查组(对照组),每组各30例,比较两组患者宫腔异常发生率、IVF-ET妊娠结局及相关激素表达情况。结果实验组宫腔正常14例,异常16例,对照组宫腔异常15例,两组比较,差异无统计学意义(P>0.05);两组患者移植胚胎数目比较,差异无统计学意义(P>0.05);实验组妊娠率及胚胎种植率明显高于对照组,差异有统计学意义(P<0.05);实验组术后ER以及PR表达水平均明显高于对照组,实验组LIF及VEGF表达水平均明显高于对照组,差异有统计学意义(P<0.05)。结论宫腔镜下子宫内膜微刺激术联合子宫内膜浆细胞检查能改善IVF-ET患者的妊娠结局,可提高患者的妊娠率及胚胎种植率,提高患者对疗效的满意度,值得临床推广。

Objective To explore the clinical effects of hysteroscopic endometrial micro stimulation combined with endometrial plasma cell examination on pregnancy outcome and prognosis in patients with in vitro fertilization and embryo transfer(IVF-ET).Methods A total of 60 patients with routine IVF-ET treated at our hospital between April 2016 and March 2018 were selected.According to whether to perform hysteroscopy and endoplasmic reticulum cell examination before the next cycle,they were divided into hysteroscopy group(experimental group)and non-hysteroscopy group(control group),with 30 cases in each group.The uterine abnormal incidence,pregnancy outcome of IVF-ET and related hormone expression between two groups were compared.Results There were 14 cases of normal uterine cavity and 16 cases of abnormal uterine cavity in the experimental group,and 15 cases of abnormal uterine cavity in the control group.There was no significant difference between the two groups(P>0.05).There was no significant difference in the number of transplanted embryos between the two groups(P>0.05).The pregnancy rate and embryo implantation rate of the experimental group were significantly higher than those of the control group(P<0.05).After surgery,the expression levels of ER and PR in the experimental group were significantly higher than those in the control group,while the expression levels of LIF and VEGF in the experimental group were significantly higher than those in the control group(P<0.05).Conclusion Hysteroscopic endometrial micro stimulation combined with endometrial plasma cell examination can improve the pregnancy outcome of IVF-ET patients,which can improve the pregnancy rate and embryo implantation rate,and improve the satisfaction of patients with the curative effect,which is worthy of clinical promotion.
