高频与低频超声结合Alaczar评分对卵巢巨大包块的诊断价值

Diagnostic value of high and low frequency ultrasound combined with Alaczar score in huge ovarian mass

目的探讨联合使用高低频超声在卵巢巨大囊性包块良恶性鉴别诊断中的价值。方法回顾性分析2018年1月~2020年1月我院经手术病理学证实的42例卵巢巨大囊性包块患者的声像图,包块最大径线≥115 mm,最小径线≥90 mm,平均径线≥95 mm,均进行高、低频超声检查。观察囊壁厚度、光滑度、乳头突起、内部回声,内部血流信号及频谱形态,采用改进的Alaczar超声评分系统对卵巢肿块进行评分,以术后病理结果为金标准。探讨高低频超声联合使用对卵巢巨大囊性肿瘤良恶性鉴别诊断的价值。结果与单独使用低频超声相比,高低频联合使用明显提高了卵巢巨大囊性包块良恶性诊断的符合率(94.44%vs 85.71%,P<0.05)。结论联合使用高低频超声在卵巢巨大囊性包块良恶性鉴别诊断中具有重要的临床价值。

Objective To explore the value of combined use of high and low frequency ultrasound in the differential diagnosis of benign and malignant ovarian giant cystic masses.Methods The sonograms of 42 patients with ovarian giant cystic mass confirmed by surgery and pathology in our hospital from January 2018 to January 2020 were retrospectively analyzed.The maximum diameter of the mass was more than 115 mm,the minimum diameter was more than 90 mm,and the average diameter was more than 95 mm.The wall thickness,smoothness,papilla process,internal echo,internal blood flow signal and frequency spectrum were observed.The value of high and low frequency ultrasound in the differential diagnosis of ovarian giant cystic tumor were analyzed.Results Compared with the use of low-frequency ultrasound alone,the combination of high and low frequency significantly improved the coincidence rate of benign and malignant diagnosis of giant ovarian cystic mass(94.44%vs 85.71%,P<0.05).Conclusion The combined use of high and low frequency ultrasound had greater clinical value in the differential diagnosis of benign and malignant ovarian giant cystic masses.
