2011—2018年上海市含麻疹成分疫苗疑似预防接种异常反应监测分析

Surveillance of adverse events following immunization with measles-containing vaccines in Shanghai,2011−2018

【目的】分析上海市含麻疹成分疫苗(MCV)预防接种的安全性。【方法】通过中国免疫规划信息管理系统收集2011—2018年上海市报告的麻疹减毒活疫苗(MV)、麻疹风疹联合减毒活疫苗(MR)、麻腮风联合减毒活疫苗(MMR)的疑似预防接种异常反应(AEFI)个案,采用描述流行病学的方法进行分析。【结果】2011—2018年上海市共报告MCV的AEFI个案共15055例,年平均报告发生率为139.74/10万;其中男女性别比为1.17∶1。MCV不良反应中,一般反应为12068例(111.70/10万剂),MV、MR和MMR分别为1050例(64.66/10万剂)、5484例(134.36/10万剂)和5534例(108.60/10万剂);异常反应为2939例(27.20/10万剂),MV、MR和MMR分别为408例(25.12/10万剂)、1983例(48.59/10万剂)和548例(10.75/10万剂);一般反应主要临床损害为发热(96.01/10万);异常反应以过敏性皮疹为主(25.00/10万剂)。【结论】上海市含麻疹成分疫苗均具有良好的安全性,但仍应加强其AEFI的监测。

[Objective]To analyze the safety of measles-containing vaccines(MCV)in Shanghai city.[Methods]Adverse event following immunization casesrelated tomeaslesattenuated livevaccine(MV),measlesand rubellacombined attenuated livevaccine(MR)and measles,mumps and rubella combined attenuated live vaccine(MMR)vaccination in Shanghai during 2011—2018 were collected from the National AEFI Surveillance System for epidemiological analysis.[Results]A total of 15055 MVC-related AEFI cases were reported,for an incidence of 139.74 per 100000 doses.Among all MVC-related AEFI cases,the male to female ratio was 1.17∶1,and 12068(111.70 per 100000 doses)were common reactions,with 1050 from MV(64.66 per 100000),5484 from MR(134.36 per 100000)and 5534 from MMR(108.60 per 100000).Most of common reactions were fever(96.01 per 100000),but 2939(27.20 per 100000)were rare reactions,with 408 from MV(25.12 per 100000),1983 from MR(48.59 per 100000)and 548 from MMR(10.75 per 100000).Most of rare reactions were allergic rash(25.00 per 100000).[Conclusion]MCV has a good safety profile but we should continue to strengthen MCV-related AEFI surveillance.
