股骨近端防旋髓内钉与InterTAN治疗高龄股骨粗隆间骨折的临床效果

Clinical Effect of Anti-rotation Intramedullary Nail of Proximal Femur and InterTAN in the Treatment of Intertrochanteric Fracture of Femur in Elderly

目的探究老年股骨粗隆间骨折采用股骨近端防旋髓内钉(PFNA)与InterTAN钉治疗的临床疗效。方法选取该院2017年9月—2020年4月收治的92例老年股骨粗隆间骨折患者为研究对象,按照随机数字表法分为两组,每组46例。对照组行PFNA内固定治疗,观察组行InterTAN钉内固定治疗,对比两组手术指标、术后不同时间髋关节功能Harris评分、并发症发生情况。结果对照组手术时间为(80.12±2.52)min,短于观察组,术中出血量为(268.52±2.28)mL,少于观察组,差异有统计学意义(t=32.919、205.630,P<0.05);观察组术后1个月髋关节Harris评分为(92.89±3.96)分,高于对照组,差异有统计学意义(t=5.639,P<0.05);观察组并发症发生率为2.17%,低于对照组,差异有统计学意义(χ^(2)=9.583,P<0.05)。结论老年股骨粗隆间骨折采用PFNA治疗具有手术时间短、术中出血量少的优点,而InterTAN钉内固定术则能改善患者术后早期髋关节功能,降低并发症发生率。

Objective To explore the clinical effects of intertrochanteric fractures of the elderly with proximal femoral anti-rotation intramedullary nails(PFNA)and inter tan nails.Methods 92 elderly patients with intertrochanteric fractures of the femur who were admitted to the hospital from September 2017 to April 2020 were selected as the research objects.They were divided into two groups according to the random number table method,with 46 cases in each group.The control group received PFNA internal fixation treatment,and the observation group received intertrochanteric fractures.The tan nail internal fixation was used to compare the surgical indicators,the Harris score of hip joint function at different times after the operation,and the occurrence of complications between the two groups.Results The operation time of the control group was(80.12±2.52)min,which was shorter than the observation group,and the intraoperative blood loss was(268.52±2.28)mL,which was less than the observation group,the difference was statistically significant(t=32.919,205.630,P<0.05);the observation group of postoperative 1-month Harris score of hip joint was(92.89±3.96)points,which was higher than that of the control group,the difference was statistically significant(t=5.639,P<0.05);the complication rate of the observation group was 2.17%,which was lower than that of the control group,the difference was statistically significant(χ^(2)=9.583,P<0.05).Conclusion The use of PFNA in the treatment of intertrochanteric fractures in the elderly has the advantages of short operation time and less intraoperative blood loss.InterTAN nail internal fixation can improve the early postoperative hip function and reduce the incidence of complications.
