绷带式角膜接触镜用于翼状胬肉手术治疗的临床评价

The Clinical Evaluation of Bandage Contact Lens for Pterygium Surgery

目的探讨绷带式角膜接触镜在翼状胬肉手术治疗中的应用价值。方法选择2019年1—12月在该院行翼状胬肉手术治疗的332例患者,均采取翼状胬肉切除术联合角膜干细胞移植术的方式进行治疗,其中44例患者在术后放置角膜绷带镜(观察组),另288例患者术后不做特殊处理(对照组),对比两组患者的临床效果。结果观察组患者在术后1 d、术后3 d以及术后5 d的眼部疼痛评分分别为(6.2±0.7)分、(3.6±0.8)分、(0.9±0.3)分,而对照组分别为(6.8±0.9)分、(4.2±1.1)分、(1.4±0.5)分,差异有统计学意义(t=4.229,3.478、6.453,P<0.05);观察组患者在术后1周的角膜上皮愈合染色评分分别为(1.9±0.4)分、(0.8±0.3)分,而对照组分别为(2.2±0.5)分、(1.1±0.4)分,差异有统计学意义(t=3.797,4.772,P<0.05);经过干预后,观察组患者的角膜上皮缺损面积为(0.2±0.1)mm^(2),明显小于对照组患者的缺损面积(0.9±0.7)mm^(2),差异有统计学意义(t=6.615,P=0.001);观察组患者的完全愈合时间为(22.9±3.1)d,明显短于对照组的完全愈合时间(27.6±3.4)d,差异有统计学意义(t=8.636,P=0.001);观察组与对照组患者的并发症发生率分别为2.3%、2.7%,其结果较为一致,差异无统计学意义(χ^(2)=0.094,P=0.759);此外,对照组中发现2例胬肉复发情况,但观察组患者在康复过程中并无复发现象出现。结论将绷带式角膜接触镜应用于翼状胬肉手术治疗后的患者中,可有效缓解其眼部疼痛程度,同时加速患者角膜上皮的愈合,在其康复及预后过程中均有积极应用价值。

Objective To explore the application value of bandage contact lens in the surgical treatment of pterygium.Methods A total of 332 patients who underwent pterygium surgery in the hospital from January 2019 to December 2019 were selected.All patients were treated with pterygium resection combined with corneal stem cell transplantation.44 patients were placed corneal bandage mirror(observation group)after surgery,another 288 patients did not receive special treatment after operation(control group),and the clinical effects of the two groups were compared.Results The eye pain scores of patients in the observation group were(6.2±0.7)points,(3.6±0.8)points,and(0.9±0.3)points on the 1st,3rd,and 5th days after the operation,respectively,while the control group were(6.2±0.7)points,(3.6±0.8)points,and(0.9±0.3)points respectively;(6.8±0.9)points,(4.2±1.1)points,(1.4±0.5)points in the control group,the difference was statistically significant(t=4.229,3.478,6.453,P<0.05);observation group of the corneal epithelial healing staining scores of the patients at 1 week after surgery were(1.9±0.4)points and(0.8±0.3)points,while those of the control group were(2.2±0.5)points and(1.1±0.4)points,respectively.The difference was statistically significant(t=3.797,4.772,P<0.05);after intervention,the corneal epithelial defect area of the observation group was(0.2±0.1)mm^(2),which was significantly smaller than that of the control group(0.9±0.7)mm^(2),the difference was statistically significant(t=6.615,P=0.001);the complete healing time of the observation group was(22.9±3.1)d,which was significantly shorter than the control group(27.6±3.4)d,the difference was statistically significant(t=8.636,P=0.001);the complication rates of the observation group and the control group were 2.3% and 2.7% respectively,and the results were relatively consistent,the difference was not statistically significant(χ^(2)=0.094,P=0.759);in addition,2 cases of pterygium recurrence were found in the control group,but no recurrence occurred in the observation group during the recovery process.Conclusion The application of bandage contact lens to patients after pterygium surgery can effectively relieve their ocular pain and accelerate the healing of the patient’s corneal epithelium.It has positive application value in the process of rehabilitation and prognosis.
