人工髋关节置换术治疗老年髋部骨折的效果评价

Evaluation of the Effect of Artificial Hip Replacement in the Treatment of Elderly Hip Fractures

目的评价人工髋关节置换术治疗老年髋部骨折的效果。方法对该院2018年1月—2019年10月收治的老年髋部骨折患者进行筛选,从中选择86例作为该次研究的对象,按照奇偶数的不同随机分为对照组(空心加压螺钉内固定治疗)、研究组(人工髋关节置换术治疗),各43例。比较两组手术指标、髋关节功能评分、疼痛程度评分、术后并发症发生率。结果对照组手术时间、下地负重时间(85.83±6.41)min、(23.07±2.81)d短于研究组(99.07±5.02)min、(35.49±2.42)d、术后引流量(93.09±23.51)mL少于研究组(120.51±12.36)mL,术后7 d对照组疼痛程度评分(1.34±0.41)分低于研究组(2.91±0.37)分,差异有统计学意义(t=10.663、21.961、6.770、18.641,P<0.05);术后1个月、3个月研究组髋关节评分(68.92±3.96)分、(85.67±3.27)分高于对照组(60.28±3.67)分、(71.51±3.19)分,差异有统计学意义(t=10.493、20.325,P<0.05);研究组术后并发症发生率2.33%低于对照组18.60%,差异有统计学意义(χ^(2)=4.468,P<0.05)。结论对老年髋部骨折患者实施人工髋关节置换术患者髋关节功能恢复更好,术后脱位等并发症发生率更低,但术后疼痛程度、术中出血量等高于内固定治疗。

Objective To evaluate the effect of artificial hip replacement in the treatment of elderly hip fractures.Methods The elderly patients with hip fracture admitted in the hospital from January 2018 to October 2019 were screened,86 cases were selected as the objects of this study,and they were randomly divided into the control group(with the hollow compression screw fixed treatment),study group(artificial hip replacement treatment)according to the odd and even numbers,43 cases in each group.The surgical index,hip joint function score,pain degree score,and postoperative complication rate were compared between the two groups.Results The operation time and weight-bearing time of the control group(85.83±6.41)min,(23.07±2.81d)d were shorter than those of the study group(99.07±5.02)min,(35.49±2.42)d,and the postoperative drainage volume(93.09±23.51)mL was less than the study group by(120.51±12.36)mL,the control group's pain degree score(1.34±0.41)points was lower than the study group's(2.91±0.37)points on the 7th postoperative da,the difference was statistically significant(t=10.663,21.961,6.770,18.641,P<0.05);1 and 3 months postoperatively of the hip scores of the study group(68.92±3.96)points,(85.67±3.27)points were higher than those of the control group at(60.28±3.67)points,(71.51±3.19)points,the difference was statistically significant(t=10.493,20.325,P<0.05);the postoperative complication rate of 2.33%in the group was lower than 18.60%in the control group,the difference was statistically significant(χ^(2)=4.468,P<0.05).Conclusion For elderly patients with hip fractures undergoing artificial hip replacement,the hip joint function recovers better,and the incidence of complications such as postoperative dislocation is lower,but the degree of postoperative pain and intraoperative blood loss are higher than that of internal fixation.
