彩色多普勒超声在下肢深静脉血栓诊断中的应用效果研究 被引量:5

摘要 目的探讨彩色多普勒超声诊断下肢深静脉血栓的效果。方法选择该院2019年1—12月收治确诊的下肢深静脉血栓患者计62例为实验组,另外选择该院2019年1—12月收治下肢无病变患者62例为对照组,所有患者均采用彩色多普勒超声进行检查,对比两组患者超声检查下肢静脉内径与血流情况。结果实验组62例患者血栓分型包括中央型35例,周围型14例,混合型13例。实验组CFV、SFV、POV、PTV、MGV值分别为(12.37±1.34)mm、(7.48±1.38)mm、(6.12±0.62)mm、(2.94±0.55)mm、(4.28±0.95)mm均大于对照组(8.91±1.62)mm、(5.35±1.02)mm、(5.85±0.76)mm、(2.56±0.53)mm、(3.55±0.41)mm,差异有统计学意义(t=12.205、9.205、2.042、3.690、5.232,P<0.05)。实验组CFV、SFV、POV的CDFI值分别为(8.54±1.54)cm/s、(5.02±1.41)cm/s、(7.20±1.33)cm/s,均小于对照组(21.75±5.41)cm/s、(12.25±3.13)cm/s、(9.47±1.50)cm/s,差异有统计学意义(t=17.417、15.619、8.398,P<0.05)。急性期彩色多普勒超声特征为管径扩张,无回声或低回声;亚急性期与慢性期管径未见明显扩张,回声增强,管腔中存在密实性低回声填充。CDFI显示管腔边缘、中央存在细条血流信号或局部断流。结论彩色多普勒超声诊断下肢深静脉血栓形成效果理想,且具有无创伤、无禁忌证、可重复等优势。 Objective To investigate the effect of color Doppler ultrasound in the diagnosis of deep vein thrombosis of the lower extremities.Methods A total of 62 patients with deep venous thrombosis of the lower extremities admitted to the hospital from January to December 2019 were selected as the experiment group,and 62 patients with no lower extremity lesions admitted in the hospital from January to December 2019 were selected as the conerol group,all patients were examined by color Doppler ultrasound,and the ultrasound examination of the inner diameter and blood flow of the lower limbs of the two groups of patients was compared.Results The thrombus types of 62 patients in the experimental group included 35 cases of central type,14 cases of peripheral type,and 13 cases of mixed type.The values of CFV,SFV,POV,PTV and MGV in the experimental group were(12.37±1.34)mm,(7.48±1.38)mm,(6.12±0.62)mm,(2.94±0.55)mm,(4.28±0.95)mm,respectively,greater than control group(8.91±1.62)mm,(5.35±1.02)mm,(5.85±0.76)mm,(2.56±0.53)mm,(3.55±0.41)mm,the difference was statistically significant(t=12.205,9.205,2.042,3.690,5.232,P<0.05).The CDFI values of CFV,SFV,and POV in the experimental group were(8.54±1.54)cm/s,(5.02±1.41)cm/s,(7.20±1.33)cm/s,respectively,which were all smaller than those of the control group(21.75±5.41)cm/s,(12.25±3.13)cm/s,(9.47±1.50)cm/s,the difference was statistically significant(t=17.417,15.619,8.398,P<0.05).Color Doppler ultrasound in the acute phase is characterized by dilation of the tube diameter,no echo or hypoechoic;no obvious dilation of the tube diameter in the subacute and chronic phases,echo enhancement,and dense hypoechoic filling in the lumen.CDFI showed that there were thin blood flow signals or local cutoff at the edge and center of the lumen.Conclusion Color Doppler ultrasound has an ideal effect in the diagnosis of deep vein thrombosis of the lower extremities,and has the advantages of non-invasiveness,no contraindications,and reproducibility.
