初产妇助产护理中导乐联合自由体位的应用分析

Application Analysis of Doula Combined Free Body Position in Primiparous Midwifery Nursing

目的分析在初产妇助产护理中选择导乐陪护联合自由体位的价值。方法以2019年4月—2020年4月98名初产妇为对象,随机数表法分为两组,对照组49名采取传统仰卧位分娩,进行常规分娩护理,导乐组49名则接受导乐陪护联合自由体位分娩,比较两组分娩质量。结果导乐组阴道自然分娩率、阴道助产率、剖宫产率分别为81.63%、12.24%、6.12%,均优于对照组51.02%、28.57%、20.41%,差异有统计学意义(χ^(2)=10.280、4.021、4.346,P<0.05);导乐组第一产程、第二产程、第三产程用时均短于对照组,差异有统计学意义(t=9.831、11.017、10.957,P<0.05);导乐组产痛评分为(6.26±2.18)分低于对照组(7.95±1.33)分,分娩控制感评分(145.82±20.39)分高于对照组(117.85±14.25)分,差异有统计学意义(t=4.633、7.871,P<0.05)。结论初产妇助产护理中选择导乐陪护联合自由体位分娩可提升分娩质量,减低剖宫产率。

Objective To analyze the value of choosing Doula escort combined free position in the nursing of primiparas.Methods From April 2019 to April 2020,98 primiparous women were divided into two groups by random number table method.The control group 49 cases were delivered in the traditional supine position and received routine delivery care,while the Doula group 49 cases received Doula escort combined with free body delivery,compared the delivery quality of the two groups.Results The vaginal natural delivery rate,vaginal delivery rate,and cesarean section rate of the Doula group were 81.63%,12.24%,and 6.12%,respectively,which were better than those of the control group by 51.02%,28.57%,and 20.41%,the difference was statistically significant(χ^(2)=10.280,4.021,4.346,P<0.05);the first,second,and third stages of labor in the Doula group were shorter than those in the control group,the difference was statistically significant(t=9.831,11.017,10.957,P<0.05);the birth pain score of the Doula group was(6.26±2.18)points lower than the control group(7.95±1.33)points,and the labor control score(145.82±20.39)points was higher than the control group(117.85±14.25)points,the difference was statistically significant(t=4.633,7.871,P<0.05).Conclusion Choosing Doula escort combined with free body delivery in the midwifery nursing of primipara can improve the quality of delivery and reduce the rate of cesarean section.
