不同频率重复经颅磁刺激治疗精神分裂症的临床疗效分析

Analysis on the Clinical Efficacy of Repetitive Transcranial Magnetic Stimulation at Different Frequencies in the Treatment of Schizophrenia

目的分析不同频率重复经颅磁刺激(rTMS)治疗精神分裂症的临床疗效。方法选择2018年1月至2020年1月我院收治的120例精神分裂症患者,随机分为三组各40例。低频组给予低频r TMS联合利培酮口服液治疗,高频组给予高频rTMS联合利培酮口服液治疗,对照组给予利培酮口服液治疗。比较三组的治疗效果。结果治疗后,三组的PANSS总分、阴性症状评分、阳性症状评分均显著降低,且高频组、低频组均显著低于对照组(P <0.05);高频组的阴性症状评分显著低于低频组,阳性症状评分显著高于低频组(P <0.05);高频组与低频组治疗后PANSS总分比较,差异无统计学意义(P>0.05)。治疗后,三组的PRL水平均显著升高,高频组的PRL水平显著低于低频组,低频组的PRL水平显著低于对照组(P <0.05)。结论低频和高频rTMS均能有效治疗精神分裂症,但低频更适合阳性症状治疗,高频更适合阴性症状治疗。

Objective To analyze the clinical efficacy of repetitive transcranial magnetic stimulation(rTMS)at different frequencies in the treatment of schizophrenia.Methods 120 patients with schizophrenia admitted to our hospital from January 2018 to January 2020 were selected and randomly divided into three groups,with 40 patients in each group.The low frequency group was treated with low frequency rTMS combined with risperidone oral solution,the high frequency group was treated with high frequency rTMS combined with risperidone oral solution,and the control group was treated with risperidone oral solution.The therapeutic effects of the three groups were compared.Results After treatment,the total PANSS score,negative symptom score and positive symptom score of the three groups decreased significantly,and the high frequency group and the low frequency group were significantly lower than the control group(P<0.05);The negative symptom score of the high frequency group was significantly lower than that of the low frequency group,while the positive symptom score was significantly higher than that of the low frequency group(P<0.05);No significant difference was found in total PANSS score between the high frequency group and the low frequency group(P>0.05).After treatment,the PRL levels of the three groups increased significantly,the PRL level of the high frequency group was significantly lower than that of the low frequency group,and the PRL level of the low frequency group was significantly lower than that of the control group(P<0.05).Conclusions Both low frequency and high frequency rTMS are effective in the treatment of schizophrenia,but low frequency is more suitable for the treatment of positive symptoms,and high frequency is more suitable for the treatment of negative symptoms.
