小儿重症支气管肺炎发生的影响因素分析

Analysis on the Influencing Factors for Occurence of Severe Bronchopneumonia in Children

目的探讨小儿重症支气管肺炎发生的影响因素。方法回顾性分析80例小儿支气管肺炎患儿的临床资料,根据疾病严重程度分为观察组(重症支气管肺炎,n=15)和对照组(非重症支气管肺炎,n=65),采用单因素和Logistic多因素分析探讨小儿重症支气管肺炎发生的影响因素。结果两组的性别比较,差异无统计学意义(P>0.05);观察组中年龄<2岁、合并佝偻病、合并先天性心脏病、存在免疫缺陷、 STAMP评分≥4分、冬春季节发病、病程≥14 d占比均高于对照组(P <0.05)。Logistic多因素分析显示,年龄<2岁、合并佝偻病、合并先天性心脏病、存在免疫缺陷、 STAMP评分≥4分、冬春季节发病、病程≥14 d是小儿重症支气管肺炎发生的危险因素(OR>1, P <0.05)。结论支气管肺炎患儿中,年龄较小,合并佝偻病、先天性心脏病,存在免疫缺陷,STAMP评分≥4分,病程长,冬春季节发病均是引发重症支气管肺炎的危险因素,临床治疗时需重点关注。

Objective To explore the influencing factors for occurence of severe bronchopneumonia in children.Methods The clinical data of 80 children with bronchiopneumonia were retrospectively analyzed and divided into observation group(severe bronchiopneumonia,n=15)and control group(non-severe bronchiopneumonia,n=65)according to the severity of the disease.Univariate and Logistic multivariate analyses were used to explore the influencing factors for occurrence of severe bronchiopneumonia in children.Results No significant difference was found in gender between the two groups(P>0.05).The observation group was higher than the control group in the proportions of age<2 years,complication of rickets,complication of congenital heart disease,complication of immunodeficiency,STAMP score≥4 points,onset in winter and spring,and disease duration≥14 days(P<0.05).Logistic multivariate analysis showed that age<2 years,complication of rickets,complication of congenital heart disease,complication of immunodeficiency,STAMP score≥4 points,onset in winter and spring,and disease duration≥14 days were the risk factors for the occurrence of severe bronchopneumonia in children(OR>1,P<0.05).Conclusions In children with bronchopneumonia,younger age,complication of rickets and congenital heart disease,complication of immunodeficiency,STAMP score≥4 points,onset in winter and spring,and long disease duration are risk factors for severe bronchial pneumonia,which should be paid more attention in clinical treatment.
