临终患者病情评估表预测恶性肿瘤终末期患者生存期的信效度研究

Study of the reliability and validity of terminal patient condition assessment form in predicting the survival period of terminal patients with malignant tumors

目的 :评价临终患者病情评估表的信效度。方法 :将上海市静安区临汾路街道社区卫生服务中心安宁疗护科2018年4月至2019年2月所有的住院恶性肿瘤终末期患者160例作为研究对象,其中在院死亡的138例。入院时用临终患者病情评估表对138例在院死亡的患者进行评估,采用Spearman-Brown系数、克朗巴赫系数评价评估表信度,采用Pearson相关性分析、探索性因子分析评价评估表效度。结果 :临终患者病情评估表的Spearman-Brown系数为0.806,克朗巴赫系数为0.778。临终患者病情评估表总分与生存期的相关系数为0.804,通过因子分析提取了3个公因子,分别为主观症状因子、客观体征因子和机体衰弱因子。结论 :临终患者病情评估表预测恶性肿瘤终末期患者生存期的信效度均较好,未来可进一步扩大样本量从而更准确地预测恶性肿瘤终末期患者的生存情况,为临床医务人员及肿瘤终末期患者提供相关决策依据。

Objective:To assess the reliability and validity of the disease assessment form of the terminal patients.Methods:From April 2018 to February 2019,160 patients with terminal stage of malignant tumors in the Hospice Department of Community Health Service Center of Linfen Community of Jingan District,Shanghai were selected as research objects,including 138 in-hospital deaths.At the time of admission,138 patients who died in the hospital were evaluated with the terminal patient condition assessment form,the reliability of the assessment form was evaluated with Spearman-Brown coefficient and Cronbach coefficient,and the validity of the assessment form was evaluated with Pearson correlation analysis and exploratory factor analysis.Results:The Spearman-Brown coefficient was 0.806 and the Kronbach coefficient was 0.778 in the evaluation form of the terminal patients.The correlation coefficient between the total score of the terminal patient condition assessment form and the survival period of the terminal patients was 0.804,three common factors were extracted by factor analysis,which were subjective symptom factor,objective physical sign factor and body weakness factor.Conclusion:The reliability and validity of the terminal patient condition assessment form to predict the survival period of patients with malignant tumors are good,and in the future,the sample size can be further expanded to more accurately predict the survival of patients with end-stage malignant tumors,to provide relevant decision-making basis for clinical medical staff and patients with terminal stage tumor.
