子宫切口妊娠病灶切除术治疗子宫下段剖宫产切口瘢痕处妊娠患者的临床效果及对血清激素水平的影响

The influence of hysterectomy on the clinical effect and serum hormone level of pregnant women with incision scar after cesarean section

目的探讨子宫切口妊娠病灶切除术治疗子宫下段剖宫产切口瘢痕处妊娠患者的临床效果及对血清激素水平的影响。方法选取2019年1月-2020年8月因子宫下段剖宫产切口瘢痕处妊娠来该院接受治疗的120例患者为研究对象,随机设为研究组与对照组。对照组采用子宫动脉灌注栓塞术治疗,研究组采用子宫切口妊娠病灶切除治疗。比较两组临床效果、治疗前后血清激素水平、性功能评分以及两组并发症发生情况。结果与对照组相比,研究组手术时间及月经恢复时间均缩短(P<0.05),两组术中出血量及住院时间比较,差异无统计学意义(P>0.05);与对照组治疗后相比,研究组β-h CG水平较低,而性功能评分较高(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论对子宫下段剖宫产切口瘢痕处妊娠患者采取病灶切除治疗,利于患者术后恢复,改善机体血清激素水平,对患者性功能影响较小,安全性较高,利于临床推广应用。

Objective To investigate the effect of hysterectomy on the clinical effect and serum hormone level of pregnant patients with incision scar after cesarean section.Methods A total of 120 patients admitted to our hospital from January 2019 to August 2020 for treatment of pregnancy due to incision scar in cesarean section of the lower uterine segment were selected as study subjects and randomly assigned to study group and control group.The control group was treated by uterine artery embolization,while the study group was treated by hysterectomy.Clinical effects,serum hormone levels and sexual function scores before and after treatment,as well as complications were compared between the two groups.Results Compared with the control group,the operation time and menstrual recovery time of the study group were shortened(P<0.05),but there was no significant difference in intraoperative blood loss and length of hospital stay between the two groups(P>0.05).Compared with the control group,the level of-h CG was lower and the sexual function score was higher(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion The treatment of the lesion excision in the lower section of the uterus cesarean section scar pregnancy is beneficial to the postoperative recovery of the patients,improve the body serum hormone levels,have little impact on the sexual function of the patients,higher safety,is conducive to clinical application.
