电子心力测量法无创心输出量监测在新生儿重症监护室中的应用 被引量:2

Application of electronic cardiometry noninvasive cardiac output monitoring in Neonatal Intensive Care Unit

摘要 目的分析电子心力测量法无创心输出量监测在新生儿重症监护室中的应用。方法随机选取2018年5月-2019年5月深圳市宝安区人民医院新生儿重症监护室(NICU)的81例危重患儿为研究组,另随机选取同期在深圳市宝安区人民医院产科分娩的81例健康足月儿为对照组。两组新生儿均应用电子心力测量法和多普勒超声心动图法测定心输出量(CO)。分析两组新生儿出生后1、7、14 d的CO变化。结果新生儿出生后1、7、14 d,两种方法测量的两组CO比较差异均无统计学意义(均P>0.05)。随时间延长,两种方法测量的两组CO均显著升高(均P<0.05)。结论无创心输出量监测在NICU中有应用价值,是一种简便、快捷、便宜、准确监测新生儿CO的方法,可提高NICU中危重症患儿的血流动力学诊治水平,电子心力测量法无创心输出量监测在NICU中的应用效果与多普勒超声心动图法相当,可在临床推广使用。 Objective To analyze electronic cardiometry noninvasive cardiac output monitoring in Neonatal Intensive Care Unit.Methods From May 2018 to May 2019,81 critically ill children were randomly selected from Neonatal Intensive Care Unit(NICU),People’s Hospital of Baoan District as study group,81 healthy full-term neonates born in Department of Obstetrics in People’s Hospital of Baoan District were randomly selected as control group.Electronic cardiometry and Doppler echocardiography were used to detect cardiac output(CO),the changes of CO at 1,7,14 days after birth in the two groups were analyzed.Results There was no statistically significant difference in CO at 1,7,14 days after birth between the two groups(P>0.05),the levels of CO in the two groups increased significantly with time(P<0.05).Conclusion Noninvasive cardiac output monitoring has application value in NICU,which is a simple,fast,economical,and accurate method to monitor neonatal CO,the method can improve diagnosis and treatment level of hemodynamics in critically ill children in NICU,the application effect of electronic cardiometry noninvasive cardiac output monitoring is similar to Doppler echocardiography in NICU,so it is worthy of clinical promotion and application.
