奥沙利铂联合氟尿嘧啶治疗晚期胃癌的近期疗效及预后

Short-term efficacy and prognosis of oxaliplatin combined with fluorouracil in the treatment of advanced gastric cancer

目的观察晚期胃癌患者采用奥沙利铂联合氟尿嘧啶治疗的近期疗效及预后。方法选取2014年1月-2017年1月福建省立医院收治的晚期胃癌患者80例,根据数字奇偶法随机分为观察组和对照组,各40例。观察采用奥沙利铂+氟尿嘧啶治疗胃癌,对照组采用顺铂+氟尿嘧啶治疗胃癌,比较2组近期疗效、用药不良反应、生活质量评分及预后。结果观察组患者近期疾病总控制率为82.50%,对照组为80.00%,组间比较差异无统计学意义(χ^(2)=0.082,P=0.775);治疗后,2组不良反应主要为中性粒细胞减少、白细胞减少、恶心呕吐、皮疹腹泻及神经毒性等,观察组不良反应总发生率为22.50%,低于对照组的50.00%(χ^(2)=6.540,P=0.011);治疗后,观察组生活质量评分为(71.24±3.55)分,高于对照组的(67.88±4.87)分(t=3.526,P=0.001);观察组癌症转移率(7.50%)、胃癌复发率(17.50%)及3年生存率(75.00%),与对照组(12.50%、15.00%、72.50%)比较差异均无统计学意义(P>0.05)。结论奥沙利铂联合氟尿嘧啶药物有效应用治疗晚期胃癌患者,可获得较好的近期疗效及预后,减少不良反应发生。

Objective To investigate the short-term efficacy and prognosis of oxaliplatin plus fluorouracil in the treatment of advanced gastric cancer.Methods 80 cases of advanced gastric cancer patients in Fujian Provincial Hospital from January 2014 to January 2017 were divided into two groups by digital parity method.The observation group(40 cases)was treated with oxaliplatin+fluorouracil;the control group(40 cases)was treated with cisplatin+fluorouracil.The short-term efficacy,adverse drug reactions,quality of life score and prognosis were compared between the two groups.Results The total disease control rate of the observation group was 82.50%,and that of the control group was 80.00%.There was no significant difference between the two groups(χ^(2)=0.082,P=0.775).After treatment,the adverse reactions of the two groups were mainly concentrated in neutropenia,leucopenia,nausea and vomiting,rash and diarrhea and neurotoxicity.The total incidence of adverse reactions in the observation group was 22.50%,which was lower than 50.00%in the control group(χ^(2)=6.540,P=0.011).The quality of life score of the observation group was(71.24±3.55),which was higher than that of the control group(67.88±4.87)(t=3.526,P=0.001).There was no significant difference in the metastasis rate(7.50%),recurrence rate of gastric cancer(17.50%)and 3-year survival rate(75.00%)between the observation group and the control group(12.50%,15.00%,72.50%,P>0.05).Conclusion Oxaliplatin combined with 5-fluorouracil is effective in the treatment of advanced gastric cancer.Compared with cisplatin+5-fluorouracil,the short-term efficacy and prognosis are consistent,but it can reduce the occurrence of adverse reactions.
