自动听性脑干诱发电位在新生儿听力筛查中的应用

摘要 目的探讨自动听性脑干诱发电位(AABR)在新生儿听力筛查中的应用效果。方法选择2018年3月—2019年3月安源区妇幼保健院门诊进行听力筛查的1616例婴儿,先接受耳声发射(OAE)初筛,筛查为阳性者出生30 d后联合AABR进行复筛。统计初筛、复筛阳性率。结果初筛阳性率为15.90%,复筛阳性率为5.45%,复筛后阳性率低于初筛,差异有统计学意义(P<0.05);6例(42.86%)确诊为听力障碍,总发病率为0.37%。结论新生儿听力筛查中应用AABR可有效减低OAE初筛时的假阳性率,提高筛查准确率,避免出现过度治疗。 Objective To investigate the application of automated auditory brainstem response(AABR)in newborn hearing screening.Methods 1616 newborns with hearing screening in the outpatient of Anyuan district maternal and child health care hospital from March 2018 to March 2019 were selected,and they underwent preliminary screening of otoacoustic emissions(OAE).The newborns with positive screening were re-screened in combination with AABR after birth for 30d.The positive rates of preliminary screening and secondary screening were recorded.Results The positive rate of preliminary screening was 15.90%and the positive rate of secondary screening was 5.45%,and the positive rate of secondary screening was lower than that of preliminary screening,with statistically significant difference(P<0.05);6 cases(42.86%)were confirmed as hearing disorder,with a total incidence of 0.37%.Conclusion The use of AABR in newborn hearing screening can effectively reduce the false positive rate at the initial screening of OAE,improve the accuracy of screening,and avoid overtreatment.
