脐血嗜酸性粒细胞趋化因子水平对早期婴儿湿疹的预测意义 被引量:3

摘要 目的分析脐血嗜酸性粒细胞趋化因子(Eotaxin)水平的影响因素,探讨脐血Eotaxin水平对42 d婴儿湿疹的预测意义。方法采集188例新生儿的脐血,应用酶联免疫吸附法测定脐血Eotaxin的水平,登记新生儿的出生情况及家族过敏史等基本信息,随访42 d婴儿的湿疹患病情况。结果家族有过敏性疾病史组的新生儿脐血Eotaxin水平明显高于家族无过敏性疾病史组(P <0.05);与无湿疹组比较,婴儿患有湿疹组脐血Eotaxin水平显著升高(P<0.05);根据logistic回归分析表明脐血Eotaxin高表达水平、家族过敏史是影响婴儿湿疹患病的独立危险因素(P<0.05);脐血Eotaxin的ROC曲线的结果为:AUC为0.773,95%CI为0.704~0.841,特异度为52.9%,灵敏度为88.8%。结论脐血Eotaxin高表达水平是影响婴儿湿疹患病的独立危险因素。脐血Eotaxin水平对婴儿湿疹的发生有一定的预测作用。 Objective To analyze the influencing factors of umbilical cord blood Eotaxin level and explore the predictive significance of umbilical cord blood Eotaxin level in 42-day-old infants eczema.Methods The umbilical cord blood was collected from 188 newborns.The level of Eotaxin in umbilical cord blood was determined by enzyme-linked immunosorbent assay(Elisa).The birth status and family history of allergies were recorded.The infants were followed up for 42 days.Results The umbilical cord blood Eotaxin level in the family history of allergic disease group was significantly higher than that in the family without allergic disease group,and the umbilical cord blood Eotaxin level in the eczema group was significantly higher than that in the non-eczema group(P<0.05).According to logistic regression analysis,the high expression level of Eotaxin in umbilical cord blood and family history of allergy were important risk factors for infantile eczema.The area under ROC curve(AUC)of umbilical cord blood Eotaxin was 0.773(95%CI:0.704~0.841),the specificity was 52.9%,and the sensitivity was 88.8%.Conclusion The high expression level of Eotaxin in umbilical cord blood is an independent risk factor for infantile eczema.Umbilical Cord blood Eotaxin level can predict the occurrence of infantile eczema.
