阴道联合经腹超声诊断疑似异位妊娠的效果及准确性分析

Analysis of the Effect and Accuracy of Vaginal Combined Transabdominal Ultrasound in the Diagnosis of Suspected Ectopic Pregnancy

目的探析阴道联合经腹超声诊断疑似异位妊娠的效果及准确性。方法选取该院2018年4月—2020年4月期间就诊的60例疑似异位妊娠患者为观察对象,均给予经腹超声、阴道超声诊断,以手术病理诊断为金标准,比较经腹超声与阴道联合经腹超声诊断的准确性、特异度、敏感度及声像图特征。结果60例患者经手术病理诊断可知,49例患者确诊为异位妊娠。经腹超声诊断的准确性、特异度、敏感度分别为81.67%、54.55%、87.76%,阴道联合经腹超声诊断准确性、特异度、敏感度分别为96.67%、90.91%、97.96%,对比差异有统计学意义(χ^(2)=6.990、4.240、4.850,P<0.05)。在胚芽形成、假孕囊、血管搏动、盆腔积液、附件包块方面,经腹超声诊断阳性率分别为4.08%、8.16%、10.20%、61.22%、71.43%,阴道联合经腹超声诊断阳性率分别为16.33%、24.49%、28.57%、81.63%、89.80%,比较差异有统计学意义(χ^(2)=4.010、4.780、5.290、5.000、5.290,P<0.05)。结论异位妊娠患者应用阴道联合经腹超声诊断的效果十分理想,具有操作简单、准确性高等优势。

Objective To explore the effectiveness and accuracy of vaginal combined with transabdominal ultrasound in the diagnosis of suspected ectopic pregnancy.Methods Sixty patients with suspected ectopic pregnancy who visited the hospital from April 2018 to April 2020 were selected as observation objects.All were diagnosed by transabdominal ultrasound and vaginal ultrasound.The surgical pathological diagnosis was used as the gold standard to compare transabdominal ultrasound and transabdominal ultrasound.The accuracy,specificity,sensitivity and sonographic characteristics of the diagnosis of vaginal combined transabdominal ultrasound.Results The pathological diagnosis of 60 patients showed that 49 patients were diagnosed with ectopic pregnancy.The diagnostic accuracy,specificity,and sensitivity of transabdominal ultrasound were 81.67%,54.55%,and 87.76%,respectively.The diagnostic accuracy,specificity,and sensitivity of vaginal combined transabdominal ultrasound were 96.67%,90.91%,and 97.96%,respectively.The difference was statistically significant(χ^(2)=6.990,4.240,4.850,P<0.05).In terms of embryo formation,pseudo-gestational sac,vascular pulsation,pelvic effusion,and adnexal mass,the positive rates of transabdominal ultrasound were 4.08%,8.16%,10.20%,61.22%,71.43%,and the positive rates of diagnosis of vaginal combined with transabdominal ultrasound were 16.33%,24.49%,28.57%,81.63%,89.80%,and the difference was statistically significant(χ^(2)=4.010,4.780,5.290,5.000,5.290,P<0.05).Conclusion The diagnosis effect of vaginal combined with transabdominal ultrasound in patients with ectopic pregnancy is very satisfactory.It has the advantages of simple operation and high accuracy.
