机械取栓和静脉溶栓治疗急性脑梗死的临床效果研究

Study on the Clinical Effect of Mechanical Thrombectomy and Intravenous Thrombolysis in the Treatment of Acute Cerebral Infarction

目的探究机械取栓和静脉溶栓治疗急性脑梗死的临床效果。方法选择该院2018年1月—2019年12月期间收治的72例急性脑梗死患者为研究对象,根据单盲分组法进行1∶1比例分组,每组36例。参照组应用静脉溶栓方式对患者进行治疗,研究组应用机械取栓方式对患者进行治疗,比较两组患者的临床疗效、临床指标等情况。结果研究组活化凝血酶原时间、凝血酶原时间、凝血酶时间、纤维蛋白原数值分别为(35.01±1.23)s、(12.12±1.54)s、(19.23±1.69)s、(2.56±0.45)g/L,优于参照组,差异有统计学意义(t=7.912、2.994、3.335、6.931,P<0.05);研究组患者NIHSS评分分别为(11.59±0.57)分、(8.47±1.14)分、(5.23±1.77)分,明显低于参照组,差异有统计学意义(t=5.611、3.284、4.276,P<0.05)。结论在急性脑梗死的临床治疗中,机械取栓相对于静脉溶栓方式的临床效果更为显著,其安全性更高。

Objective To explore the clinical effects of mechanical thrombus removal and intravenous thrombolysis in the treatment of acute cerebral infarction.Methods 72 cases of patients with acute cerebral infarction admitted to the hospital from January 2018 to December 2019 were selected as the research objects.According to the single blind grouping method,they were divided into 1∶1 groups with 36 cases in each group.The control group was treated with intravenous thrombolysis,and the study group was treated with mechanical thrombectomy.The clinical efficacy and clinical indicators of the two groups were compared.Results The activated prothrombin time,prothrombin time,thrombin time and fibrinogen value of the study group were(35.01±1.23)s,(12.12±1.54)s,(19.23±1.69)s and(2.56±0.45)g/L respectively,which were better than those of the control group,the difference was statistically significant(t=7.912,2.994,3.335,6.931,P<0.05);the NIHSS scores of the study group were(11.59±0.57)points,(8.47±1.14)points and(5.23±1.77)points,which was significantly lower than that of the control group,the difference was statistically significant(t=5.611,3.284,4.276,P<0.05).Conclusion In the clinical treatment of acute cerebral infarction,mechanical thrombectomy has a more significant clinical effect than intravenous thrombolysis,and its safety is higher.
