双Endobutton钢板固定术治疗急性肩锁关节脱位效果的临床观察

Clinical observation of the effect of double Endobutton plate fixation in the treatment of acute acromioclavicular joint dislocation

目的:探究关节脱位者应用双Endobutton钢板固定术治疗的效果。方法:回顾性分析2018.1-2019.12期间本院收治的84例肩锁关节脱位患者资料,锁骨钢板治疗组41例为对照组,双Endobutton钢板固定术组43例为研究组,对两组患者的手术时间、住院时间进行比较,同时对比术中出血量和术中切口大小,并评估术前术后疼痛视觉模拟评分(VAS)和肩关节Constant-Murley评分(CMS评分),并比较两组并发症情况。结果:两组患者的手术时间与术中出血量比较,差异无统计学意义(p>0.05);对照组患者术中切口和住院时间为6.53±0.42cm和6.35±0.38d,观察组为4.53±0.45cm和4.72±0.36d,观察组小于对照组,差异有统计学意义(p<0.05)。术前,两组患者VAS评分和CMS评分比较,差异无统计学意义(p>0.05);术后,对照组VAS评分为2.16±0.31,观察组为0.84±0.32,两组患者VAS均较术前降低且观察组降低更显著,差异有统计学意义(p<0.05);术后,对照组CMS评分为72.96±8.41,观察组为93.41±8.32,差异有统计学意义(p<0.05)。对照组并发症几率为9.76%,观察组为2.33%,观察组并发症几率低于对照组,差异有统计学意义(p<0.05)。结论:全关节镜下双Endobutton钢板固定术治疗肩锁关节脱位可减少术中切口缓解患者疼痛,促进关节恢复并改善预后,降低并发症几率,临床疗效可观。

Objective:To explore the effect of dual Endobutton plate fixation for patients with joint dislocation.Methods:A retrospective analysis of the data of 84 patients with acromioclavicular joint dislocation admitted to our hospital from 2018.1-2019.12,41 patients in the clavicle plate treatment group were the control group,and 43 patients in the double Endobutton plate fixation group were the study group.The operation on the two groups of patients Time and hospital stay were compared,intraoperative blood loss and intraoperative incision size were compared at the same time,and the visual analogue scale(VAS)of preoperative and postoperative pain and the Constant-Murley score(CMS score)of the shoulder joint(CMS)were evaluated,and the complications of the two groups were compared.Happening.Results:There was no significant difference between the operation time and intraoperative blood loss between the two groups of patients(p>0.05);the intraoperative incision and hospitalization time of the control group were 6.53±0.42cm and 6.35±0.38d,and the observation group was 4.53±0.45cm and 4.72±0.36d,the observation group was smaller than the control group,the difference was statistically significant(p<0.05).Before operation,the difference between the two groups of patients'VAS score and CMS score was not statistically significant(p>0.05);after the operation,the VAS score of the control group was 2.16±0.31,and the observation group was 0.84±0.32.The former decreased and the observation group decreased more significantly,the difference was statistically significant(p<0.05);after the operation,the CMS score of the control group was 72.96±8.41,and the observation group was 93.41±8.32,the difference was statistically significant(p<0.05).The complication rate of the control group was 9.76%,and that of the observation group was 2.33%.The complication rate of the observation group was lower than that of the control group,and the difference was statistically significant(p<0.05).Conclusion:Full arthroscopic double Endobutton plate fixation for the treatment of acromioclavicular joint dislocation can reduce intraoperative incision to relieve patient pain,promote joint recovery and improve prognosis,reduce the probability of complications,and have considerable clinical efficacy.
