高尿酸血症对早期糖尿病肾病患者肾功能影响的研究

Study on the Effect of Hyperuricemia on Renal Function in Patients with Early Diabetic Nephropathy

目的探讨高尿酸血症对早期糖尿病肾病患者肾功能影响。方法方便选择2017年5月—2020年4月该院收治的70例早期糖尿病肾病患者作为研究对象,以是否存在高尿酸血症将其分为两组,其中36例伴有高尿酸血症的患者为研究组,34例未合并高尿酸血症的患者为对照组。两组患者均行空腹血糖(FPG)、尿酸(SUA)、肌酐(Scr)、尿素氮(Blood urea nitrogen,BUN)检测,并计算肾病指数与尿白蛋白排泄率(UAER)。对比两组患者FPG与肾功能(SUA、SCr、24 h UAER、BUN、肾病指数)指标,分析SUA与FPG、24 h UAER、SCr、BUN、肾病指数的相关性。结果研究组SUA(480.52±46.63)μmol/L、24 h UAER(188.52±35.52)mg/24 h、BUN(23.35±5.63)mmol/L与肾病指数(240.52±45.00)均高于对照组(315.52±30.52)μmol/L、(135.65±33.45)mg/24 h、(11.56±3.82)mmol/L与(64.08±20.00),差异有统计学意义(t=17.406,6.454,10.193,20.983,P<0.001);36例高尿酸血症患者中,SUA水平与24 hUAER、BUN、肾病指数呈正相关性(r=0.166,0.400,0.430,P<0.001)。结论高尿酸血症对于早期糖尿病肾病患者的肾功能具有不良影响,临床应及时采取降尿酸治疗措施,以便改善患者的预后。

Objective To investigate the effect of hyperuricemia on renal function in patients with early diabetic nephropathy.Methods Convenient select Seventy patients with early diabetic nephropathy admitted to the hospital from May 2017 to April 2020 were selected as the research objects,and they were divided into two groups based on whether there was hyperuricemia.Among them,36 patients with hyperuricemia as the study group,34 patients without hyperuricemia as the control group.Both groups of patients were tested for fasting blood glucose(FPG),uric acid(SUA),creatinine(Scr),and blood urea nitrogen(BUN),and the nephropathy index and urinary albumin excretion rate(UAER)were calculated.The FPG and renal function(SUA,SCr,24 h UAER,BUN,nephropathy index)indexes were compared between the two groups of patients,and the correlation between SUA and FPG,24 h UAER,SCr,BUN,and nephropathy index was analyzed.Results The study group's SUA(480.52±46.63)μmol/L,24 h UAER(188.52±35.52)mg/24 h,BUN(23.35±5.63)mmol/L and nephropathy index(240.52±45.00)were all higher than those of the control group(315.52±30.52)mmol/L,(135.65±33.45)mg/24 h,(11.56±3.82)mmol/L and(64.08±20.00),the difference was statistically significant(t=17.406,6.454,10.193,20.983,P<0.001);Among 36 patients with hyperuricemia,SUA levels were positively correlated with 24 hUAER,BUN,and nephropathy index(r=0.166,0.400,0.430,P<0.001).Conclusion Hyperuricemia has an adverse effect on the renal function of patients with early diabetic nephropathy.Clinical measures should be taken to reduce uric acid in time to improve the prognosis of patients.
