慢性乳腺炎患者分离克氏棒杆菌的耐药性及临床意义研究

摘要 目的探讨克氏棒杆菌在慢性乳腺炎发病中的作用及耐药性。方法收集2018年9月至2020年3月该院从慢性乳腺炎患者中分离的克氏棒杆菌19株,采用E-实验法进行药敏试验;采用1%吐温-80哥伦比亚血平板培养细菌;探索慢性乳腺炎伴克氏棒杆菌感染患者的合适的治疗方法。结果19株克氏棒杆菌对万古霉素、利奈唑胺、庆大霉素、利福平敏感率均达到100.0%,对青霉素、环丙沙星、四环素、美罗培南敏感率分别为20.0%、30.0%、10.0%、60.0%;对红霉素和克林霉素耐药率均为100.0%。19株克氏棒杆菌在1%吐温-80哥伦比亚血平板上生长良好。结论克氏棒杆菌在慢性乳腺炎的发生、发展中具有重要作用,准确、快速检出该菌和药敏试验结果对患者的诊治具有重要意义。 Objective To explore the effects and drug resistance of C.kroppenstedtii in the pathogenesis of chronic mastitis.Methods Nineteen strains of C.kroppenstedtii isolated from the patients with chronic mastitis in this hospital from September 2018 to March 2020 were collected;the drug sensitivity test was carried out by adopting the E-test method and the bacteria were cultured by adopting 1%Tween-80 Colombian blood plate;the appropriate therapeutic method in the patients with chronic mastitis complicating C.kroppenstedtii infection was investigated.Results The sensitivity rates of 19 strains of C.kroppenstedtii to vancomycin,linezolid,gentamicin and rifampicin all reached 100.0%,which to penicillin,ciprofloxacin,tetracycline and meropenem were 20.0%,30.0%,10.0%and 60.0%respectively;the resistance rates to erythromycin and clindamycin all were 100.0%.Nineteen strains of C.kroppenstedtii grew well on Colombian blood plate with 1%Tween-80.Conclusion C.kroppenstedtii has an important role in the occurrence and development of chronic mastitis.Accurate and rapid detection of this bacterium and drug susceptibility test results have an important significance in the diagnosis and treatment of the patients.
