生肌玉红膏联合活血化瘀中药离子导入对糖尿病足患者足部血流、创面愈合的影响

Effect of Shengji Yuhong Ointment Combined with Iontophoresis of Traditional Chinese Medicine to Promote Blood Circulation and Remove Blood Stasis on Foot Blood Flow and Wound Healing in Diabetic Foot Patients

目的探讨生肌玉红膏外敷联合活血化瘀中药离子导入对糖尿病足(diabetic foot,DF)患者足部血流、创面愈合及临床疗效的影响。方法选取2018年6月—2020年9月本院DF患者117例,按照随机数字表法分为观察组和对照组。对照组58例在西医常规治疗基础上予生肌玉红膏外敷,观察组59例在对照组治疗基础上联合活血化瘀中药离子导入治疗。比较2组患者临床疗效、创面愈合情况、足部血流情况及细胞因子水平。结果治疗后,观察组患者临床治疗总有效率为91.53%,显著高于对照组的74.14%(P<0.05)。观察组患者创面愈合时间显著短于对照组,创面面积显著小于对照组,创面愈合率显著高于对照组(P<0.05)。观察组患者踝肱指数、足背动脉平均血流速度显著高于对照组,足背动脉管腔内径显著宽于对照组(P<0.05)。观察组患者血清肿瘤坏死因子(tumor necrosis factor,TNF)-α、白细胞介素(interleukin,IL)-1β、晚期糖基化终末产物(advanced glycation end products,AGEs)水平显著低于对照组(P<0.05);碱性成纤维细胞生长因子(basic fibroblast growth factor,bFGF)、血管内皮生长因子(vascular endothelial growth factor,VEGF)水平显著高于对照组(P<0.05)。结论生肌玉红膏外敷联合活血化瘀中药离子导入治疗DF患者疗效确切,可降低炎症因子水平、促进血管内皮生长、改善足背血流、加速创面愈合,进而提高临床疗效。

Objective To investigate the effects of external application of Shengji Yuhong ointment combined with iontophoresis of traditional Chinese medicine to promote blood circulation and remove blood stasis on foot blood flow,wound healing and clinical efficacy in diabetic foot(DF)patients.Methods From June 2018 to September 2020,one hundred and seventeen DF patients were selected and divided into observation group and control group according to random digital table method.In the control group,58 cases were treated with Shengji Yuhong ointment on the basis of routine western medicine treatment,while 59 cases in the observation group were treated with iontophoresis of traditional Chinese medicine to promote blood circulation and remove blood stasis on the basis of control group.The clinical efficacy,wound healing,foot blood flow and cytokine levels were compared between the two groups.Results After treatment,the total effective rate of clinical treatment in the observation group was 91.53%,which was significantly higher than that in the control group(74.14%)(P<0.05).The wound healing time in the observation group was significantly shorter than that in the control group,the wound area was significantly smaller than that in the control group,and the wound healing rate was significantly higher than that in the control group(P<0.05).The ankle-brachial index and mean blood flow velocity of dorsal foot artery in the observation group were significantly higher than those in the control group(P<0.05).The levels of serum tumor necrosis factor(TNF)-α,interleukin(IL-1β),advanced glycation end products(AGEs)in the observation group were significantly lower than those in the control group(P<0.05).The levels of serum basic fibroblast growth factor(bFGF)and vascular endothelial growth factor(VEGF)in the observation group were significantly higher than those in the control group(P<0.05).Conclusion External application of Shengji Yuhong ointment combined with iontophoresis of traditional Chinese medicine to promote blood circulation and remove blood stasis was effective in the treatment of DF patients,which could reduce the level of inflammatory factors,promote the growth of vascular endothelial,improve the blood flow of the dorsal foot,accelerate the wound healing,and thus improve the clinical efficacy.
