晚期胃癌患者未满足需求现状调查及其影响因素分析

Investigation on Unmet Demand of Patients with Advanced Gastric Cancer and Its Influencing Factors

目的调查晚期胃癌患者未满足需求现状及其影响因素。方法便利选取2018年3月—2019年5月在某三级综合公立医院就诊的269名晚期胃癌患者参与研究,使用基本情况问卷、多维领悟社会支持量表、Connor-Davidson心理弹性问卷中文简化版和汉化癌症患者未满足需求简明量表进行调查,使用多元线性回归方法对未满足需求的影响因素进行统计分析。结果254例晚期胃癌患者未满足需求得分为(80.80±10.25)分,领悟社会支持得分为(57.28±11.85)分,心理弹性得分为(32.75±10.24)分。多元线性回归结果提示,性别、患癌时间、已治疗费用、家庭人均收入、初诊患癌、领悟社会支持和心理弹性均是晚期胃癌患者未满足需求的显著影响因素(P<0.05)。结论晚期胃癌患者未满足需求程度较高,护士应向患者及家属告知晚期胃癌治疗和护理方案,指导患者在晚期胃癌的治疗。患者出院后,指导患者家属对期进行延续性护理,与患者进行交流,适时地对其进行心理护理,改善患者未满足需求。

Objective To investigate the unmet needs of patients with advanced gastric cancer and its influencing factors.Methods 269 patients with advanced gastric cancer who were hospitalized in our hospital from March 2018 to May 2019 were conveniently selected to participate in the study.Basic Situation Questionnaire,Multidimensional Perceived Social Support Scale,Connor-Davidson Psychological Elasticity Questionnaire,Chinese Simplified Version and Chinese Simplified Scale of Unmet Needs of Cancer Patients were used to investigate them.Multiple linear regression was used to analyze the influencing factors of unsatisfied demands.Results The score of unsatisfactory needs was(80.80±10.25),the score of perceived social support was(57.28±11.85)and the score of resilience was(32.75±10.24)in 254 patients with advanced gastric cancer.Multivariate linear regression showed that gender,time of cancer,cost of treatment,per capita family income,newly diagnosed cancer,perceived social support and resilience were all significant factors influencing unmet needs of patients with advanced gastric cancer(P<0.05).Conclusion The patients with advanced gastric cancer have a higher degree of unsatisfactory needs,and nurses should inform patients and their families of the treatment and nursing plan of advanced gastric cancer,and guide patients in the treatment of advanced gastric cancer.After the patient discharging from the hospital,the family members of the patient shall be instructed to carry out continuous nursing,communicating with the patient,timely carrying out psychological nursing,and improving their unsatisfied needs.
