摘要
目的检测微小RNA-137(miR-137)、微小RNA-351(miR-351)在妊娠期糖尿病(GDM)患者血清中的表达情况,探讨其对妊娠结局的影响。方法选取2017年2月至2019年8月产检并分娩的GDM孕妇138例(观察组)为研究对象,根据妊娠结局分为良好组(70例)和不良组(68例);选取同期健康孕妇140例(对照组)为对照。采用化学发光法检测空腹血糖(FPG)、空腹血清胰岛素(FINS)水平,计算胰岛素抵抗指数(HOMA-IR);用全自动生化分析仪检测血清总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平;采用qRT-PCR法检测血清miR-137、miR-351水平;采用Pearson法分析GDM孕妇血清miR-137、miR-351水平与其他临床指标的相关性;采用COX回归分析影响GDM孕妇妊娠结局的因素。结果观察组孕妇FINS、FPG、HOMA-IR、血清TC、TG、LDL-C、miR-137、miR-351水平较对照组均显著升高(P<0.05);不良组GDM孕妇FINS、FPG、HOMA-IR、血清TC、TG、LDL-C、miR-137、miR-351水平较良好组均显著升高(P<0.05);血清miR-137水平与FINS、FPG、HOMA-IR、血清TC、TG、LDL-C水平,血清miR-351水平与FINS、血清TC、TG、LDL-C水平均显著正相关(P<0.05);FPG、血清TG、miR-137水平为影响GDM孕妇妊娠结局的独立危险因素(P<0.05)。结论血清miR-137、miR-351参与GDM发生发展,可影响孕妇体内糖脂代谢,其中,血清miR-137水平可能影响GDM孕妇妊娠结局。
Objective To investigate the effects of the expressions of microRNA-137(miR-137)and microRNA-351(miR-351)on the pregnancy outcome of of patients with gestational diabetes mellitus(GDM).Methods A total of 138 pregnant women with GDM who were delivered in our hospital from February 2017 to August 2019 were enrolled as observation group,according to the pregnancy outcome,who were divided into good outcome group(n=70)and poor outcome group(n=68),moreover,the other 140 healthy pregnant women were enrolled as control group.The levels of fasting blood glucose(FPG)and fasting serum insulin(FINS)were measured by chemiluminescence,and HOMA-IR was calculated,moreover,the serum levels of total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)were detected by automatic biochemical analyzer,and the serum levels of miR-137 and miR-351 were detected by qRT-PCR.And Pearson method was used to analyze the correlation between serum miR-137,miR-351 and the other clinical indexes,and COX regression analysis was used to analyze the influencing factors on the pregnancy outcome of pregnant women with GDM.Results The serum levels of FINS,FPG,HOMA-IR,TC,TG,LDL-C,miR-137 and miR-351 in observation group were significantly higher than those in control group(P<0.05),moreover,which in poor outcome group were significantly higher than those in good outcome group(P<0.05).In addition the serum levels of miR-137 were positively correlated with those of FINS,FPG,HOMA-IR,TC,TG and LDL-C,and the serum levels of miR-351 were positively correlated with those of FINS,TC,TG and LDL-C(P<0.05).Furthermore the serum levels of FPG,TG and miR-137 were independent risk factors influencing the pregnancy outcome of patients with GDM(P<0.05).Conclusion The srrum miR-137 and miR-351 are involved in the occurrence and development of GDM,which may affect the glycolipid metabolism of pregnant women,in which,the serum levels of miR-137 may affect the pregnancy outcome of pregnant women with GMD.
