多学科团队协作模式在养老机构老年慢性病患者长期护理服务中的应用

Application of multidisciplinary team cooperation model in long-term care service for elderly patients with chronic diseases in elderly care institutions

目的探讨多学科团队协作护理模式在养老机构老年慢性病患者长期护理服务中的应用效果。方法选取泸州社会福利院老年慢性病患者120例为研究对象。组建多维度、多机构、跨专业的多学科团队,采用多学科团队协作模式进行3个月的长期护理服务。分别在实施多学科团队协作模式前后,采用问卷调查评估患者满意度,采用焦虑自评量表(SAS)和抑郁自评量表(SDS)评定患者心理状态,采用日常生活能力Barthe指数评估量表评估患者日常生活能力,采用美国医学结局研究组健康状问卷(MOSSF-36)评估患者生活质量情况。结果多学科团队协作模式实施后,老年慢性病患者的满意度评分、日常生活能力评分和生活质量评分均高于实施前(P均<0.05),患者心理状态评分均低于实施前(P均<0.05)。结论多学科团队协作模式可以提高养老机构老年慢性病患者的满意度,改善患者的心理状态,提高患者的日常生活能力和生活质量,值得推广应用。

Objective Explore the application effect of multidisciplinary team cooperation model in long-term care services for elderly patients with chronic diseases in elderly care institutions.Methods A total of 120 elderly patients with chronic diseases in Luzhou Social Welfare Institute were selected.Establish a multi-dimensional,multi-institutional,and multi-disciplinary multidisciplinary team,and adopt a multi-disciplinary team collaboration model to provide long-term care services for 3 months.Before and after the implementation of the multidisciplinary teamwork model,questionnaire surveys were used to assess patient satisfaction,the Self-Rating Anxiety Scale(SAS)and the Self-Rating Depression Scale(SDS)were used to assess the mental state of patients,and the Barthe Index of Daily Life Ability Assessment Scale was used to assess the patient’s ability of daily living,the American Medical Outcome Research Group Health Status Question⁃naire(MOSSF-36)was used to assess the patient’s quality of life.Results After the implementation of the multidisci⁃plinary teamwork model,the satisfaction score,the ability of daily living score and the quality of life scores of elderly pa⁃tients with chronic diseases were all higher than before(all P<0.05),and the mental state scores of the patients were all lower than before(all P<0.05).Conclusion The multidisciplinary team cooperation model can improve the satisfaction of elderly patients with chronic diseases,improve the mental state of the patients,and improve the ability of daily living and the quality of life of the patients,which is worthy of promotion and application.
