涤痰汤加减治疗冠心病心绞痛有效性及安全性的Meta分析

Efficacy and safety of Ditan Decoction in the treatment of angina pectoris of coronary heart disease

目的评价涤痰汤加减治疗冠心病心绞痛的有效性及安全性。方法计算机检索PubMed、Cochrance Library、EMbase、Mdeline、Sinomed、CNKI、Wanfang Data、VIP等数据库,纳入相关临床随机对照试验,采用RevMan软件分析数据。结果纳入11篇文献,涉及患者1346例。主要结局指标:①西雅图心绞痛量表:试验组与对照组疗后相比,平均降低2.05。次要结局指标:①GRACE评分:试验组与对照组疗后相比,平均降低1.23。②心电图:试验组与对照组疗后相比,心电图改善有效率高于对照组约1.23。③中医症状疗效:试验组与对照组疗后相比,胸痛评分降低1.62、胸闷评分降低1.64、心悸评分降低2.37、身体困乏评分降低3.69。④不良事件发生情况:由于所纳入研究数量及报告不良事件的限制,涤痰汤加减的安全性尚需要进一步临床试验验证。结论涤痰汤加减可在一定程度上改善患者心绞痛发作及心肌缺血,提高临床预后,缓解症状如胸痛、胸闷、心悸和身体困乏,为冠心病心绞痛临床治疗提供一定参考。

Objective To evaluate the efficacy and safety of Ditan Decoction in the treatment of angina pectoris of Coronary Heart Dsease.Methods PubMed,Cochrane Library,EMBASE,mdeline,sinomed,CNKI,Wanfang Data,VIP and other databases were retrieved by computer and included in the related clinical randomized controlled trials.Revman software was used to analyze the data.Results 11 articles were included,involving 1346 patients.Main outcome measures were based on Seattle Angina scale.The experimental group was 2.05 lower than the control group.Secondary outcome indicators:①Grace Score:Compared with the control group,the average decrease in the experimental group was 1.23.②ECG:Compared with the control group,the effective rate of ECG improvement in the experimental group was about 1.23 higher than that in the control group.③TCM symptom effect:Compared with the control group,the experimental group has a 1.62-lower chest pain score,a 1.64-lower chest distress score,a 2.37-lower palpitation score,and a 3.69-lower body fatigue score.④Occurrence of adverse events:Due to the limitation of the number of studies included and adverse events reported,the safety of Ditan Decoction addition and subtraction still needed to be further verified by future clinical trials.Conclusion Ditan Decoction can improve angina pectoris,myocardial ischemia and the clinical prognosis,relieve the symptoms such as chest pain,chest distress,palpitation and physical sleepiness,and provide some reference for the clinical treatment of angina pectoris of coronary heart disease.
