肺炎支原体肺炎喘息患儿潮气呼吸肺功能变化 被引量:6

Changes of Tidal Breathing Lung Function in Wheezing Children with Mycoplasma Pneumoniae Pneumonia

摘要 目的:研究1~48月龄肺炎支原体肺炎(mycoplasma pneumoniae pneumonia, MPP)喘息患儿肺功能在急性期受损情况以及恢复期的恢复情况。方法:收集自2019年1月至2020年12月在我院住院的诊断为MPP并完成潮气呼吸肺功能检测患儿共152例,根据肺部听诊有无哮鸣音将患儿分为喘息组和无喘息组,其中喘息组患儿68例,无喘息组患儿84例。随机选取同期、同年龄、性别比接近,近2月无呼吸道感染病史的30例住院患儿作为对照组,进行潮气呼吸肺功能检测,并在1月后随访患儿肺功能。结果:急性期喘息组和非喘息组患儿较对照组患儿TPTEF/TE、VPTEF/VE、Ti/Te值降低(P<0.05);喘息组TPTEF/TE、VPTEF/VE较无喘息组低(P<0.05);而恢复期复查肺功能指标TPTEF/TE、VPTEF/VE值较发作期均有明显改善(P<0.05),但较正常对照组仍偏低(P<0.05);且复查喘息组TPTEF/TE及VPTEF/VE较无喘息组低(P<0.05)。结论:肺炎支原体感染后患儿肺功能出现损伤,并且有喘息患儿较无喘息患儿肺功能损伤重,在临床恢复期复查肺功能仍未完全恢复正常,表明MP感染后患儿存在持续的气道受损和延迟恢复情况。 Objective To study the damage of lung function in the acute phase and the recovery of mycoplasma pneumoniae pneumonia(MPP)wheezing children aged 1 to 48 months.Methods A total of 152 children with mycoplasma pneumoniae pneumonia who were hospitalized in our hospital from January,2019 to December,2020 and were tested for tidal breathing lung function were collected,the children were divided into wheezing group and non-wheezing group.There were 68 chil⁃dren in the wheezing group and 84 children in the non-wheezing group.A total of 30 hospitalized children with the same age and sex ratio and without any history of respiratory infection in recent 2 months were randomly selected as the control group to test the tidal breathing lung function.Children′s lung function was followed up one month later.Results The TPTEF/TE,VPTEF/VE,Ti/Te values of children in the acute wheezing group and non-wheezing group were lower than those in the control group(P<0.05).TPTEF/TE and VPTEF/VE in the wheezing group were lower than those in the non-wheezing group(P<0.05),while the pulmonary function index TPTEF/TE and VPTEF/VE values of the recovery period were signif⁃icantly improved compared with the acute period(P<0.05),but it was still lower than the normal control group(P<005).TPTEF/TE and VPTEF/VE in the wheezing group were lower than those in the non-wheezing group(P<0.05).Conclu⁃sion The lung function of children with MP infection was damaged,and the lung function of children with wheezing was more serious than that of children without wheezing.The lung function of children with MP infection did not completely re⁃turn to normal in the clinical recovery period,which indicated that the children with MP infection had sustained airway damage and delayed recovery.
