静脉用药调配中心审方药师对促进合理用药的作用分析

Analysis of the role of pharmacists in the center of the intravenous drug compounding center in promoting rational drug use

目的研究静脉用药调配中心审方药师对促进合理用药的作用分析。方法选取2019年6月~2020年6月住院的患者医嘱中不良医嘱300组进行分析。依据时间段不同分为对照组(2019年下半年)与观察组(2020年上半年),各150组。对照组给予一般药学服务干预。观察组药方均通过静脉用药调配中心审方药师严格审核后使用静脉注射药品。比较各月份两组患者药品超量、配伍禁忌、用法错误、使用溶媒不当、医嘱错误,抗菌药物、化疗药物、普通药品、TNP处方与高风险处方、中风险处方、低风险处方数量变化。结果观察组药物不合理处方数比对照组少,观察组不合理处方类型如药品超量、配伍禁忌、用法错误、使用溶媒不当、医嘱错误发生率明显低于对照组;观察组不合理处方中抗菌药物、化疗药物、普通药品和TPN处方数量、高风险处方数、中风险处方数、低风险处方数均低于对照组。结论静脉用药调配中心经药师审方可有效降低医嘱不合理应用,提高静脉用药的有效率,减少药物的重复使用,合理调配药物,提高药物疗效,减少不良药物事件的发生,缓解医患关系,降低处方药的风险,提高患者生命质量。

Objective To study the effect of prescription pharmacists of of the intravenous drug compounding center on the promotion of rational drug use.Methods A total of 300 groups of the number of bad doctor orders among patients hospitalized from June 2019 to June 2020 in our hospital were selected for analysis.According to the time period,it is divided into a control group(the second half of 2019)and an observation group(the first half of 2020),each with 150 groups.The control group was given general pharmaceutical care intervention.The prescriptions of the observation group were used intravenously after being strictly reviewed by the prescription pharmacist of the intravenous drug compounding center.Compare the changes in the number of drug overdose,contraindications,usage errors,improper use of solvents,incorrect doctor orders,antibacterial drugs,chemotherapy drugs,general drugs,TNP prescriptions and high-risk prescriptions,medium-risk prescriptions,and low-risk prescriptions between the two groups in each month.Results The number of unreasonable prescriptions of drugs in the observation group was less than that in the control group.The types of unreasonable prescriptions in the observation group,such as overdose of drugs,contraindications,incorrect usage,improper use of solvents,and wrong doctor orders,were significantly lower than the control group;the number of antibacterial drugs,chemotherapeutics,common drugs and TPN prescriptions,high-risk prescriptions,medium-risk prescriptions,and low-risk prescriptions in reasonable prescriptions were lower than those of the control group.Conclusion The prescription approved by the pharmacist of the intravenous drug compounding center can effectively reduce the unreasonable use of doctor's orders,improve the effective rate of intravenous drugs,reduce the repeated use of drugs,rationally deploy drugs,improve the efficacy of drugs,reduce the occurrence of adverse drug events,and ease the doctor-patient relationship.Reduce the risk of prescription drugs and improve the quality of life of patients.
