基于中医传承计算平台探究宋立群教授治疗糖尿病肾病的中药组方规律

Medication Rules of DKD Treated by Professor SONG Liqun Based on TCM Inheritance Compute Platform

目的:基于中医传承计算平台(V3.0),挖掘宋立群教授治疗糖尿病肾病的用药规律及辨治经验,总结其学术思想。方法:收集肾病门诊2016年1月—2020年1月宋立群教授治疗DKD首诊处方,建立医案数据库,运用频次统计分析,相关规则分析,聚类分析等数据挖掘方法,探讨导师组方规律和辨治经验。结果:202张处方涉及用药180味,总使用频次3375次,常用药物31味,药物以补虚药为主,其次为收涩药、利水渗湿药;四气统计温性药最多,平性药次之;药味使用数最多的是甘味药和苦味药;归经统计肾经、肝经、脾经的药物最常用;常用药物组合138个,聚类分析结合临床经验得到常用特色药对5组。结论:宋立群教授治疗DKD肝脾肾同调,补虚扶正并泻实祛邪,兼以解郁、祛痰、化瘀,以期为今后的中医肾病临床提供参考。

Objective:To excavate professor SONG Liqun’s medication rules and his experience in treating diabetic kidney disease(DKD),based on TCM Inheritance Compute Platform(3.0).Methods:Collecting the first diagnosing prescriptions in the outpatient treatment of patients with DKD from Jan.2016 to Jan.2020,and establishing the medical records database,using frequency statistical analysis,related rule analysis,cluster analysis and other data mining methods to explore the prescription rules and diagnosing and treating experience of professor SONG.Results:There were 202 prescriptions involved 180 Chinese medicinal,with the total frequency of 3,375 times.There were 31 commonly used medicinal,which were mainly tonic medicinal,followed by astringent medicinal and damp-resolving medicinal.Medicinal with warm feature was mostly used followed by medicinal with mild feature.Medicinal with sweet flavor and bitter flavor accounted for the most.Medicinal that went into Kidney meridian,Liver meridian and Spleen meridian were the top three mostly used drugs.There were 138 commonly used drug combinations,and there were 5 pairs of medicinal obtained by cluster analysis combined with clinical experience.Conclusion:Professor SONG Liqun will focus on the harmonization of Liver,Spleen and Kidney in the treatment of DKD,which tonifies deficiency to support vital-qi and dispels excess to remove stagnation,phlegm and stasis.This study provided reference for the future clinical treatment of DKD.
