养血祛风汤治疗血虚风热型慢性荨麻疹疗效及对免疫因子水平的影响

Effect of Yangxue Qufeng Decoction on Chronic Urticaria of Blood Deficiency and Wind-Heat Type and Its Influence to Levels of Immune Factors

目的:探究养血祛风汤治疗血虚风热型慢性自发性荨麻疹的临床疗效及对免疫因子水平的影响。方法:96例临床确诊的慢性自发性荨麻疹患者应用随机数字表随机分成两组,治疗组口服中药养血祛风汤,对照组口服奥洛他定片治疗。分别于治疗前,治疗后4周、8周评估荨麻疹活动性评分(UAS)、免疫因子水平、临床疗效和安全性等;并于12周(停药后4周)后随访痊愈患者的复发率。结果:两组治疗后4周、8周UAS评分较治疗前均有所降低(P<0.05),治疗组UAS评分均高于对照组(P<0.05);治疗8周后,治疗组患者血清IgE、IL-4及LT水平降低幅度明显优于对照组,血清IFNγ水平升高幅度亦明显优于对照组(P<0.05);治疗4周后治疗组和对照组有效率分别为57.78%(16/45)和73.91%(34/45),两组差异无统计学意义(P>0.05);治疗8周后治疗组和对照组有效率分别为77.78%(35/45)和84.78%(39/45),两组差异无统计学意义(P>0.05);12周(停止治疗后4周)随访时治疗组复发率为12.00%(3/25);对照组复发率71.43%(20/28),对照组复发率明显高于治疗组,差异有统计学意义(P<0.05)。结论:中药养血祛风汤治疗血虚风热型慢性自发性荨麻疹安全、有效并且复发率低,可以调节患者免疫功能,值得临床推广应用。

Objective:To investigate the effect of Yangxue Qufeng Decoction on chronic urticaria of blood deficiency and wind-heat type and its influence to immune factors.Methods:96 patients of clinically confirmed chronic spontaneous urticaria were randomly divided into the treatment group and the control group.The control group was treated with Olopatadine Tablets,whereas the treatment group was also treated with Yangxue Qufeng Decoction.The urticaria activity score(UAS),immune factor,clinical efficacy and safety were evaluated before the treatment and 4 and 8 weeks after the treatment respectively.The recurrence rate of the cured patients was followed up after 12 weeks(4 weeks after drug withdrawal).Results:UAS scores at 4 and 8 weeks after the treatment in both groups were lower than those before the treatment(P<0.05);UAS scores in the treatment group were higher than those in the control group(P<0.05).After 8 weeks of the treatment,the levels of IgE,IL-4 and LT were lower,and the level of IFNγwas significantly higher in the treatment group than those in the control group(P<0.05).After 4 weeks of the treatment,the effective rate was 57.78%(16/45)in the treatment group and 73.91%(34/45)in the control group,there was no statistical difference between the two groups(P>0.05).After 8 weeks of the treatment,the effective rate was 77.78%(35/45)in the treatment group and 84.78%(39/45)in the control group,and there was no statistical difference between the two groups(P>0.05).During the follow-up(4 weeks after stopping treatment),the recurrence rate of the treatment group was 12.00%(3/25),which was significantly lower than 71.43%(20/28)of the control group(P<0.05).Conclusion:Yangxue Qufeng Decoction is safe and effective in the treatment of chronic spontaneous urticaria of blood deficiency and wind-heat type,with its low recurrence rate.It can regulate the immune function of the patients and is worthy of clinical application.
