住院糖尿病患者精准化药学服务模式的构建与实施

Construction and Implementation of Pharmaceutical Care Mode for Hospitalized Diabetic Patients under Precision Medicine System

目的:探讨精准医疗形势下,如何构建临床药师对住院糖尿病患者药学服务的模式。方法:结合药物治疗管理理念,构建本院精细化药学服务模式用于临床实践,通过个体化多个用药案例体以现药学服务的效果。结果:对本院住院糖尿病患者精细化药学服务,针对不同患者进行个体化用药服务,提高了药物治疗效果,达到了精准用药。结论:目前药师对糖尿病药学服务的流程已初步形成,标准化的工作模式有利于药学服务的规范性和可操作性。

Objective:To explore the pharmaceutical care mode for hospitalized diabetic patients provided by clinical pharmacists under a precision medicine system.Methods:Based on the way of Medication Therapy Management,a precision pharmaceutical care mode was constructed through clinical practice,and its effect was reflected by individual cases.Results:Through the implementation of pharmaceutical care mode and the provision of individualized medication services for different hospitalized diabetic patients,effect of drug treatment was improved and precision medication was achieved.Conclusions:The pharmaceutical care mode for diabetes patients provided by pharmacists in our hospital has been formed at present,which will be conducive to pharmacists'work in future.
