冠心病发生及脂代谢紊乱与血浆致动脉硬化指数的相关性研究 被引量:3

摘要 目的探讨血浆致动脉硬化指数(AIP)与冠心病相关性及性别差异。方法选取2017年1月至2019年10月在本院心血管内科接受冠状动脉造影术的受检者1012例,按检测结果分为冠心病组(n=418)和正常对照组(n=594),比较两组AIP、血脂、冠心病危险因素及与冠心病的相关性,并进一步比较是否具有性别差异。结果冠心病组高血压史、糖尿病史、吸烟史、TC、TG、LDL-C、LCI、AI、AIP均明显高于正常对照组(P<0.01),HDL-C明显低于正常对照组(P<0.01);以AIP为因变量的多元逐步回归分析显示,性别、TC、TG、HDL-C均具有相关性(P<0.05)。男性冠心病患者TC、HDL-C、LDL-C低于女性,差异有统计学意义(P<0.05),TG低于女性,差异无统计学意义,不同性别患者各指标Logistic回归分析显示,AIP与男、女冠心病患者OR值分别为2.886、7.702(P<0.001)。结论AIP反映冠心病脂代谢紊乱,可作为冠心病的预测指标,优于其他单项及综合血脂指标,且其与女性冠心病患者的相关性更密切,对冠心病发病风险评估及降血脂治疗需依据患者性别合理使用。 Objective To investigate the correlation between plasma arteriogenic index(AIP)and coronary heart disease and gender difference.Methods A total of 1012 patients who received coronary angiography in the Department of Cardiology of our hospital from January 2017 to October 2019 were selected.According to the test results,they were divided into CHD group(n=418)and normal control group(n=594).AIP,blood lipid,CAD risk factors and their correlation with CHD were compared between the two groups,and the gender difference was further compared.Results The history of hypertension,diabetes,smoking,TC,TG,LDL-C,LCI,AI and AIP in CHD group were significantly higher than those in normal control group(P<0.01),and the HDL-C was significantly lower than that in the normal control group(P<0.01).Multiple stepwise regression analysis with AIP as the dependent variable showed that gender,TC,TG and HDL-C were correlated(P<0.05).The TC,HDL-C and LDL-C of male CHD patients were lower than that of female patients,and the difference was statistically significant(P<0.05).The TG of male CHD patients was lower than that of female patients,and the difference was not statistically significant.Logistic regression analysis of various indicators of patients with different genders showed that the OR values of AIP for male and female CHD patients were 2.886 and 7.702,respectively(P<0.001).Conclusion AIP reflects lipid metabolism disorder of CHD and can be used as a predictor of CHD,which was better than other single and comprehensive lipid indexes,and was more closely correlated with female CHD patients.The risk assessment of CHD and the treatment of lowering blood lipid should be rationally used according to the gender of the patients.
