骨髓细胞形态检验在非霍奇金淋巴瘤骨髓侵犯情况的准确性评价

摘要 目的探讨骨髓细胞形态检验在非霍奇金淋巴瘤(NHL)骨髓侵犯情况的准确性评价。方法选取本院2016年5月至2019年5月收治的疑似NHL患者120例,以骨髓活检结果作为金标准,判断以骨髓细胞形态检验评定NHL骨髓侵犯情况的准确性。结果骨髓活检检测显示,120例患者NHL骨髓侵犯阳性38例,阴性82例,阳性预测率为81.48%(22/27),阴性预测率为82.79%(77/93),骨髓细胞形态检验的灵敏度为57.89(22/38),特异度为93.90%(77/82),其中检测侵袭性NHL骨髓侵犯的灵敏度为75.00%(12/16),特异度为91.30%(21/23),惰性NHL骨髓侵犯的灵敏度为54.55%(12/22),特异度为98.31%(58/59)。结论骨髓细胞形态检验法诊断NHL骨髓侵犯,准确率较高,可用于骨髓活检的补充,为临床诊断提供依据。 Objective To evaluate the accuracy of bone marrow cell morphology test in bone marrow invasion of non Hodgkin’s lymphoma(NHL).Methods From May 2016 to May 2019,120 cases patients with suspected NHL were selected in our hospital.Bone marrow biopsy results were taken as the gold standard to judge the accuracy of bone marrow cell morphology test to evaluate the bone marrow invasion of NHL.Results Bone marrow biopsy showed that there were 38 positive cases and 82 negative cases in 120 patients with NHL bone marrow invasion,the positive predictive rate was81.48%(22/27),the negative predictive rate was 82.79%(77/93),the sensitivity of bone marrow cell morphology test was 57.89(22/38),the specificity was 93.90%(77/82),the sensitivity and specificity were 75.00%(12/16)and 91.30%(21/23)for invasive NHL and 54.55%(12/22)and 98.31%(58/59)for indolent NHL.Conclusion The accuracy of bone marrow cell morphology test in the diagnosis of NHL bone marrow invasion is high,which can be used to supplement bone marrow biopsy and provide basis for clinical diagnosis.
